Our client, an established healthcare organization is seeking a Consultant Pharmacist to support long-term care and retirement homes throughout Eastern Ontario.

Key Responsibilities
- Conduct comprehensive medication reviews and pharmacotherapy assessments
- Participate in interdisciplinary care conferences
- Collaborate with physicians, nurses, and other healthcare professionals
- Identify, prevent, and resolve drug therapy issues
- Provide recommendations to optimize medication use and resident outcomes
- Support medication safety and quality improvement initiatives
- Ensure compliance with professional standards and provincial regulations
- Provide education and guidance to healthcare staff as required
- Travel to long-term care and retirement home sites within the assigned territory
Qualifications
- Licensed Pharmacist in good standing with the Ontario College of Pharmacists
- Experience in long-term care, geriatrics, retirement homes, or clinical pharmacy is considered an asset
- Strong clinical judgment and medication management skills
- Excellent communication and relationship-building abilities
- Valid driver's license and access to a vehicle
- Ability to work independently and manage multiple sites
- Strong organizational and time-management skills
