Position Summary:
Journey Home is a program for 6 adult males who have been given a Not Criminally Responsible Disposition (NCR). Staff are onsite, 24 hours a day, 7 days per week, and provide supervision and support in a safe, stable environment to help residents gradually readjust to the community after living in a secure hospital setting. This Program is delivered in partnership with the FACS NCR Treatment Team.
The Community Liaison/Recreation Worker will help clients safely and gradually readjust to the community after living in a secure hospital setting. They are also responsible for the day-to-day operation of the Home, teaching basic living skills and facilitating community activities and outings.
Responsibilities:

Goal setting with the clients that are specific to risk factors and compliment their special conditions of release

Delivery of services in a client-centred, strength-based approach

Completion of accurate written documentation, data collection/entry, intakes and assessments

Contribute to the management of high risk, high needs client base by being a member of multidisciplinary team

Ensure services are being delivered in accordance with Agency Policies and Procedures

Contribute to a positive culture by assisting and supporting colleagues as needed, and encourage and teach others about the unit’s operations

Maintain a safe, secure and clean environment

Participate in Unit Meetings, Agency Meetings, Case Conferences
Qualifications / Skills:
-
Degree or Diploma in a human services field with related experience

Valid First Aid/CPR
-
Valid Class 5 Driver’s License (No GDL)

Strong communication and pro-social relationship building skills
-
Demonstrated ability to communicate effectively with individuals with mental health conditions

Knowledge of the mental health population

Experience working with a multidisciplinary team environment

Knowledge of local activities and events is an asset

A Criminal records check is required. A criminal record does not automatically disqualify applicants; each application is evaluated individually
Conditions of Employment:

This is a position that requires frequent standing, walking up and down stairs, and occasional lifting.

Days of employment are determined according to client needs and the successful candidate must be available to work 16-18 hours per week during afternoons, evenings, and weekends.
