Yaletown House is a non profit care facility for seniors with complex care needs and is located in Vancouver, BC. We provide quality health care and services to 127 senior residents in a safe, supportive and home like environment.
Under the direction of the Registered Nurse / Team Leader, implements and evaluates nursing care for assigned residents to promote quality of life in accordance with the competency guidelines and standards of practice as outlined by the BC College of Nurses and Midwives (BCCNM) and current legislative scope of practice.
DUTIES & RESPONSIBILITIES:
- Assesses and documents according to facility guidelines, the physical, psychosocial and cognitive status of residents and consults with the Clinical Practice Manager and Registered Nurse / Team Leader as necessary. Communicates and liaises with the interdisciplinary team in providing holistic care to the residents.
- Collaborates with the Registered Nurse / Team Leader to assess, plan, implement and evaluate care for assigned residents, including reporting significant changes in a resident’s condition to Registered Nurse / Team Leader. Participates as a member of the interdisciplinary team by taking part in the development, implementation, assessment and evaluation of resident care plans and profiles including regular review and updating according to facility policies and procedures.
- Prepares the necessary documentation for resident care conferences. Attends conferences according to assigned residents as scheduled.
- Works collaboratively with Nursing Staff, in planning resident care as required and helping to set priorities in order to ensure quality of care and the safety and well-being of residents.
- Assists residents with or fully providing care as needed to meet activities of daily living i.e. nutritional needs, personal hygiene, care of skin, hair and nails, bathing and toileting, dressing, overseeing prescribed exercise routines.
- Administers and records medications according to competency guidelines and standards of practice as per the BC College of Nurses and Midwives (BCCNM).
- Identifies and reports changes (expected and unexpected) in resident status to the appropriate members of the interdisciplinary team.
- Transfers, ambulates and porters residents utilizing mechanical aides such as lifts, wheelchairs and walkers as outlined in residents care plan and as per facility policies.
- Participates and assists with resident social activities, working collaboratively with the recreation and rehabilitation team as necessary.
- Participates in the admission, discharge and transfer of residents according to facility policies and procedures.
- Advocates for residents and families to protect and promote their rights to autonomy, respect, privacy, dignity and access to information and adjusting decisions to meet the resident health needs by evaluating and identifying actual or potential limitations and strengths
- Participates, at the direction of the Director of Resident Care and/or the Clinical Practice Manager, in continuous quality improvement activities within the facility by collegially sharing information and problem solving.
- Performs other related duties as assigned.
QUALIFICATIONS:
(1) Education, Training and Experience
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Current full practicing licensure with the BC College of Nurses and Midwives (BCCNM).
- CRP-C & AED Certification.

BCNU (NBA) Wage Grid Rates apply.
