Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including pediatrics, seniors, and individuals with special needs such as dementia or hospice care. The ideal candidate will be committed to delivering personalized care while adhering to all safety and regulatory standards.
Responsibilities
- Administer medications and treatments accurately according to physician orders
- Monitor patient vital signs and observe changes in condition
- Assist with patient assessments and document findings in electronic health records such as Epic or Cerner systems
- Provide wound care, phlebotomy, and other clinical procedures as needed
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Educate patients and families on health management and post-discharge care instructions
- Maintain a safe, clean, and organized environment in compliance with healthcare regulations
- Support patients with activities of daily living in settings such as assisted living or hospice care
- Work with children, seniors, or patients with complex conditions like dementia or pain management needs
Skills
- Proven experience in nursing home, hospital, or acute care settings
- Strong knowledge of medical terminology and medication administration protocols
- Experience with electronic health records systems such as Epic or Cerner is preferred
- Familiarity with specialized care areas including dementia, pediatrics, hospice, or senior care
- Excellent patient care skills with a compassionate approach
- Ability to perform laboratory procedures such as phlebotomy when required
- Effective communication skills for working with patients of all ages and collaborating within healthcare teams
- Knowledge of case management principles and pain management techniques is advantageous
- Ability to adapt quickly to different clinical environments and handle multiple priorities efficiently
Pay: $22.00-$33.00 per hour
