OVERVIEW
The incumbent will be responsible for facilitating the timely transfer of admitted patients from the ED to the most appropriate in-patient unit to meet their care needs. Working in collaboration with supervisors, nurses, interdisciplinary teams and physicians, the Patient Flow Coordinator will ensure that barriers to discharge are removed or minimized expeditiously and that bed utilization is optimized. This role will work in conjunction with and support the Patient Flow Policy and other supporting policies as required.
EDUCATION
- BScN (or working towards it), or a Degree in another health related field required.
- All regulated Health Professionals must be in good standing with their professional organization
- Demonstrated commitment to on-going learning and professionaldevelopment.
QUALIFICATIONS
- Minimum of five years clinical experience in an acute care setting
- Demonstrated resource allocation and planning experience, specifically the coordination and delivery of services
- Knowledge of community agencies (Long Term Care, Home and Community supports etc.)
- Demonstrated leadership skills
- Excellent knowledge of organizational functions and processes
- Excellent written and verbal communication and interpersonal skills
- Display confidence and courage
- Proven critical thinking skills
- Visible, approachable and dedicated to excellence
- Ability to work effectively as a team member
