DESCRIPTION:
The Nurse Practitioner (NP) is a Registered Nurse in the Extended Class with advanced education and clinical training who provides comprehensive, patient-centered care across the continuum of health services. As the Most Responsible Provider (MRP), the NP-MRP carries full responsibility for autonomously assessing, diagnosing, ordering and interpreting diagnostic tests, initiating treatment plans, prescribing medications, and discharging patients within their legislated scope of practice. The NP-MRP holds full responsibility for coordinating and managing patients’ care, ensuring timely transitions, and facilitating appropriate follow-up.
Functioning within a framework of advanced nursing practice, the NP-MRP may collaborate with members of the interprofessional team to optimize patient outcomes through evidence-informed clinical decision-making, proactive discharge planning, and care coordination. The NP-MRP consults with physicians as appropriate (e.g. for specialist care or transfer of care if patient care needs fall outside the NP-MRP scope). The NP-MRP’s practice also includes significant
contributions to education, leadership, and research/quality improvement initiatives. The role requires consistent application of professional standards, best practice guidelines, and regulatory requirements, while actively contributing to strategic priorities related to patient flow, service excellence, and interprofessional collaboration.
In addition, the NP-MRP acts as a clinical leader and mentor, supports continuous learning for staff and students, and engages in system-level initiatives that enhance the quality of care and nursing practice. The NP-MRP is expected to model professional conduct, champion innovation, and advocate for patients and families while supporting organizational goals and maintaining compliance with relevant legislation and hospital policies.
Within this role the employee is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.
PRIMARY RESPONSIBILITIES & DUTIES:
Clinical Domain
The NP-MRP works autonomously to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the legislated scope of practice.
- Assess, analyze and synthesize a wide range of information using academic evidence and advanced clinical knowledge to develop a patient plan of care.
- Holds overall accountability for a patient’s care and participates in timely patient flow planning, preventing delays by collaborating other members of the interprofessional team and leadership as appropriate.
- Manages patients autonomously within their scope of practice and is responsible for arranging transfer of care to a physician when the patient’s care exceeds the clinical scope of the NP-MRP.
- Serves as the primary point of contact for the patient, their family, and the care team. The NP-MRP ensures continuity of care is maintained during patient transitions in care, such as discharge, and will ensure appropriate follow-up is coordinated
Leadership Domain
The NP-MRP will identify risk related to clinical practice which is managed through observation, assessment, and discussion with the involved interprofessional team members.
- Improve clinical practice through the development, implementation and evaluation of practice guidelines, standards, policies and procedures that promote excellence in nursing practice in collaboration with key stakeholders.
- Participates in clinical and organizational committees
- Provide leadership by supporting and fostering evidence-based practice and continuous quality improvement processes within the organization.
- Acts as a resource to individuals and groups within the Program and throughout the organization by promoting and ensuring excellence in nursing practice.
- Provides leadership and consultancy in professional practice to other nursing colleagues within and outside the organization locally and regionally, and to other members of the interprofessional care team.
- Advocates for individuals, families, groups and communities in relation to treatment, the health-care system and policy decision that affect health and quality of life.
- Identifies learning needs of nurses and other members of the interprofessional team and will address those needs within resources.
Research/Quality Improvement Domain
The NP-MRP will ensure that patient care and nursing practice innovations within the Program are supported by research, and are reflective of, and congruent with, evidence based or current best practice guidelines/standards of practice.
- Participates in and utilizes quality improvement/research to improve patient outcomes, contributes to the expansion of nursing knowledge, and advances the profession of nursing.
- Participate in the evaluation of current practices at the individual and systems levels identifying gaps in practice and areas for future research to enhance the standard of care and nursing practice.
Education Domain
The NP-MRP serves as a clinical resource, educator, and mentor, supporting the interprofessional team, advanced practice learners, and community partners through education, preceptorship, and collaborative consultation.
- Acts as a resource, coach and mentor to the interprofessional team; particularly related to their area(s) of expertise.
- Acts as a preceptor and resource to advanced practice nursing students and may be cross appointed at recognized academic partner institutions.
- Identifies learning needs and uses adult education concepts in planning, designing and implementing educational strategies.
- Provides consultation to community partners to support collaborative practice.
BASIC QUALIFICATIONS:
- Current registration with the College of Nurses of Ontario as Registered Nurses in the Extended Class with one of the following specialty classes: Adult, Pediatric, Neonatal, or Primary Health Care.
- Masters of Science (Nursing) or equivalent education
- Minimum 3 years of clinical nursing experience in related area.
- Previous work experience as a MRP - Nurse Practitioner (preferred)
- Membership with a professional organization such as CNA, RNAO or NPAO.
- Each NP maintains and continually improves his/her competence by participating in the College of Nurses of Ontario’s Quality Assurance requirements.
- CNA Specialty in area of primary focus (an asset).
- Evidence of exceptional clinical decision-making and critical thinking skills
- Strong knowledge of Nurse Practitioner competencies
- Demonstrated leadership skills within the interprofessional care team
- Excellent interpersonal, communication, presentation and documentation skills
- Demonstrated ability to work both autonomously and collaboratively with all team members
- Evidence of strong mentoring, teaching, team building, change leadership, organizational and prioritization skills, with the ability to work well in stressful situations
- Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care
- Exceptional time management, organizational and prioritization skills
- Demonstrated ability to problem solve and make evidence-based decisions
- Demonstrated ability to interpret/participate in research
- Demonstrated knowledge of computer database, data analysis and presentation packages required

PHYSICAL REQUIREMENTS:
The physical demand will vary dependent on the specific unit or service. The incumbent will be required to meet the physical demands described for that unit of services.
