About the Role
Working within your full legislated scope of practice, you will provide comprehensive primary health care services with a focus on health promotion, disease prevention, chronic disease management, and evidence-based care.
You will play an integral role in delivering high-quality, patient-centered care while supporting positive health outcomes across the lifespan.
Key Responsibilities
- Provide comprehensive primary care including assessment, diagnosis, treatment, and follow-up
- Conduct health assessments and develop individualized care plans
- Order and interpret laboratory and diagnostic tests, including imaging, ECGs, and ultrasounds
- Prescribe medications and therapies in accordance with NP regulations
- Manage acute, chronic, and stable health conditions using evidence-based approaches
- Deliver health education, counseling, and disease prevention initiatives
- Promote wellness and support patients in achieving their health goals
- Maintain accurate and timely documentation within the Electronic Medical Record (EMR) system
- Provide care during regular clinic hours and scheduled extended hours as required
- Collaborate effectively with interdisciplinary team members to ensure quality patient care
Qualifications
- Bachelor of Science in Nursing (BScN) required
- Master of Science in Nursing (MScN) preferred
- Current Certificate of Registration with the College of Nurses of Ontario (RN-EC)
- Membership with RNAO and NPAO
- Minimum two years of clinical experience in family medicine or a related ambulatory setting preferred
- New graduates are welcome to apply
- Strong assessment, diagnostic, clinical decision-making, and leadership skills
- Knowledge of evidence-based primary care, health promotion, and chronic disease management
- Excellent communication, organizational, and interpersonal skills
- Proficiency with EMR systems and Microsoft Office applications
- Commitment to ongoing professional development and lifelong learning
- Flexible availability, including occasional evenings and weekends
