The Service Coordinator will provide scheduling support to the In-Home Services team. The Service Coordinator will utilize scheduling software administrative tasks related to the operation of programs and will assist/liaison with staff for the implementation and maintenance of all services.
ACCOUNTABILITIES
Client and Family Centric Care Delivery
- Plans, organizes, and schedules the service delivery for clients including such services as homemaking, personal care, medication assistance, grocery shopping, transportation, and security checks.
- Completes client intake for the In-home Support department, receives referrals through HPG as needed including pre-screener assessments.
- Establishes a working rapport with clients that supports their dignity, self-direction and that facilitates their scheduling support needs.
- Supports clients with medication reminders related to taking their medications at the scheduled time(s) and in accordance with their physician’s instructions, as required.
Planning and Organizing
- Schedules personal support workers to accommodate new client referrals or to change from existing schedules, and to back-fill staff absences.
- Reconcile and complete payroll information for PSWs.
- Inputs and maintains case notes and regularly reviews clients’ Care Plans, complete documentation.

- Creates, maintains, and updates client files.
Customer Services
- Provides customer service support to clients and their caregivers and assists in navigating Lumacares support services and community resources as required.
- Manage inquiries by providing information to callers and direct phone calls when necessary
- Support department coverage including Home at last, Meals on Wheels reception, as needed.
Collaboration
- Collaborates with Team, Service Coordinators, Care Navigators and Supervisor to ensure scheduling and planning of personal support service are accurate and appropriate
- Assists clients/caregivers in connecting clients to internal and external resources, as appropriate.
- Documents and informs Supervisor of any incidents or emergency situations
- Promotes a collaborative work environment among staff and supports team building activities
- Participate in Team Meetings and Lumacare events and Townhalls
- Collaborates with our Home and Community Care, Toronto Community Housing Corporation and other external partners as necessary.
- Participate in internal and external committees/meetings, training sessions
Administration and other tasks
- Supports staff in the delivery of safe and effective services which meet client care needs,
- Committed to quality improvement and upholding organization standards.
- Maintains knowledge, observes, practices and enforces appropriate procedures in regards to safety, quality and environmental standards.
- Provides support on Statutory Holidays, weekends and evenings as needed.
- Performs other duties as required
SCHEDULE
- 37.5 hours per week
- Monday to Friday: 8:00 AM - 4:00 PM or 830-430 for the first 3 months then transition to a rotational schedule which will include Saturdays and Sundays 3-11.
- Floater role requires flexibility and the ability to work the hours required by the organization
- Role will be hybrid after the first three months of in-person training, then move to 2 days in office per week.
ROLE REQUIREMENTS
- Bachelor’s degree in health administration or related Health services degree, Medical Administrative Certificate or relevant education and work experience.
- Experience in customer service, staff scheduling and in payroll required.
- Experience working with vulnerable populations, older adults with Alzheimer’s Disease and related Dementias, disabilities, mental health, addictions, and other special needs.
- Demonstrates excellent knowledge and skill in customer service, person centered care and scheduling software.
- Demonstrates integrity, accountability and commitment to continuous improvement.
- Strong organizational and time management skills.
- Excellent computer skills, including proficiency in word processing, spreadsheet, and database applications (Word, Excel, Outlook, Microsoft Suite, Teams).
- Knowledge of Medical Terminology and Alayacare software considered an asset
- Immunization for tuberculosis, chickenpox, mumps, and hepatitis B.
- Current Vulnerable Sector Police check required completed within last 12 months is required.
- This position requires you to be fully vaccinated against COVID-19 and you must provide Lumacare with proof of vaccination prior to employment.
