Job Overview
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including pediatrics, senior care, and acute care environments. This role involves collaborating with physicians and other healthcare professionals to deliver high-quality medical services, manage patient cases, and support overall health and wellness initiatives. The Nurse Practitioner will utilize their expertise in medical terminology, medication administration, and patient assessment to ensure optimal outcomes for diverse patient populations.
Responsibilities
- Conduct thorough patient assessments, including history-taking and physical examinations
- Diagnose and treat acute and chronic illnesses across different age groups, with a focus on pediatrics and senior care
- Manage medication administration and monitor patient responses to treatments
- Develop individualized care plans in collaboration with physicians and other healthcare team members
- Perform procedures such as phlebotomy, laboratory testing, and pain management interventions
- Provide patient education on health maintenance, disease prevention, and medication usage
- Document all patient interactions accurately using electronic health records systems such as Epic or Cerner
- Coordinate case management activities to ensure continuity of care in settings like nursing homes, assisted living facilities, and home care environments
- Support dementia care initiatives by implementing specialized strategies tailored to cognitive impairments
- Assist in laboratory procedures and interpret results to inform clinical decisions
- Maintain compliance with healthcare regulations and standards of practice
Qualifications
- Valid Nurse Practitioner license with current state certification
- Experience working with children, seniors, or in hospital settings is highly desirable
- Background in nursing home, assisted living, or home care environments preferred
- Proficiency in medical terminology, medication administration, and patient care documentation systems such as Epic or Cerner
- Knowledge of acute care procedures, pain management techniques, and case management practices
- Skills in pediatric care (pediatrics), dementia care, laboratory procedures (phlebotomy), and laboratory experience preferred
- Strong communication skills with the ability to collaborate effectively within multidisciplinary teams
- Ability to manage multiple priorities while maintaining attention to detail in fast-paced settings

Pay: Up to $125,477.00 per year
