Job Overview
We are seeking a Nurse Practitioner with experience treating and managing cardiovascular disease to join our team.
This role involves collaborating with physicians and healthcare professionals to deliver high-quality medical services, manage patient treatment and respond to patient concerns. The Nurse Practitioner will utilize their expertise in medical terminology, medication administration, case management, and specifically, cardiovascular disease.
Responsibilities
- Conduct thorough patient assessments, including history taking and physical examinations. Diagnose illnesses and develop personalized treatment plans. Identify opportunities for other prevention and implement interventions.
- Prescribe medications and monitor patient responses.
- Manage chronic conditions such as hypertension, cornary artery disease, heart failure, cardiomyopathy, arrhythmia.
- Collaborate with multidisciplinary teams to coordinate case management and ensure continuity of care.
- Document all patient interactions accurately using electronic health records systems like Cerebrum EMR
- Educate patients and families on health management strategies.
Experience
- Proven experience working at cardiology clinic is highly desirable.
- Demonstrated ability to handle patient care across diverse clinical scenarios
- Familiarity with medical terminology and medication administration protocols
- Prior use of electronic health records systems.
- Strong case management skills with the ability to coordinate complex patient needs effectively.
- Excellent communication skills for working with patients of all ages and collaborating with healthcare teams.
