Job Summary
We are seeking a compassionate and dedicated Community Support Worker to assist individuals in achieving greater independence and well-being within their communities. The ideal candidate will provide personalized support, foster positive relationships, and help clients navigate daily challenges. Responsibilities
- Offer one-on-one support to clients with diverse needs, including assistance with daily living activities, such as personal care, meal preparation, and household tasks.
- Develop and implement individualized support plans tailored to each client's goals and needs.
- Facilitate access to community resources, services, and activities to promote social inclusion and engagement.
- Monitor client progress and document interactions accurately in accordance with organizational policies.
- Advocate for clients’ rights and ensure their safety and well-being at all times.
- Collaborate with families, healthcare providers, and other stakeholders to coordinate care and support services.
- Provide emotional support, encouragement, and motivation to empower clients toward greater independence.

- Excellent interpersonal and communication skills to build trusting relationships with clients from diverse backgrounds.
- Strong organizational skills to manage multiple cases effectively while maintaining detailed documentation.
- Compassionate attitude with a genuine desire to help others improve their quality of life.
- Ability to work independently as well as part of a multidisciplinary team.
- Flexibility to adapt to changing client needs and work environments.
- Basic knowledge of community resources, mental health, or social services is preferred but not required.
- Valid driver’s license is a plus for supporting clients in various community settings. This position is vital for fostering positive change within the community by supporting individuals in reaching their personal goals and enhancing their overall quality of life.
Pay: $24.00-$27.00 per hour
