Job Description
Position Summary
Responsible for completion of all Nutrition Referrals ordered by physicians nursing or multi-disciplinary team. Screen and classify nutritional risk and/or risk for malnutrition according to pre-established criteria. Provide consultative and education services to patients physicians learners other health care providers and the public. The purpose of the Registered Dietitian's job is to reduce nutrition related complications that impact hospital length of stay morbidity mortality and/or progression of an acute or chronic medical condition (i.e. Diabetes Kidney Disease).
Qualifications
- Bachelor's Degree
- 1 to 3 years' experience
- Accredited post graduate Dietetic Internship Masters Degree or equivalent (approved by College of Dietitians of Ontario). Successful completion of Canadian Dietetic Registration Examination. Membership in College of Dietitians of Ontario is mandatory.
- Membership in specialized practice groups highly recommended (examples: Association of Nephrology Dietitians Canadian Diabetes Association Society of Parenteral and Enteral Nutrition). Membership in Dietitians of Canada and/or the American Dietetic Association is optional.
- Demonstrated knowledge sensitivity and expertise in the care of seniors and their families.
- Gentle Persuasive Approach training preferred.
Responsibilities
ASSESSMENT - Collection and interpretation of biochemical anthropometric pharmaceutical data dietary medical and psychosocial history. Data is collected from medical record patient caregiver/significant other and health care team. Prescribe and/or recommend nutritional requirements including type and amount of protein carbohydrate fat calories fibre fluid vitamin and minerals. Analyze Physician Diet Order and make recommendations. Determine a Nutrition Diagnosis based on Standardized Nutrition Terminology or Department/Program standards.
PATIENT CARE PLAN - Recommend coordinate and/or implement a nutrition care plan such as a modified diet insulin adjustment (if medical directive in place) education tube/parenteral feeding. If appropriate delegate the implementation of the Nutrition Care Plan to the Dietetic Assistant or Food Service employee. Monitor and adjust care plans based on Department/Program Standards of Care and/or caseload volume. Communicate care plan in the written/electronic health record.
EDUCATION/COUNSELLING - Apply adult education techniques to develop group and individual education programs. Provide patient and/or caregiver with appropriate teaching materials that are geared to reading level readiness to change and are culturally appropriate. If suitable refer patients to additional community resources/linkages for follow up nutrition care.
PROFESSIONAL DEVELOPMENT - Develop and implement a plan for continuing education based on assessment of strengths areas of improvement short and long term goals and results of performance appraisals. Set goals with Regulated College (College of Dietitians of Ontario). Maintain expertise in Nutrition Management by attending conferences workshops medical rounds and conducting journal reviews.
RESEARCH - Participate in all aspects of Quality Assurance Projects. Work under direction of Physician for formal Research projects. When delegated collect data for research projects.
ADMINISTRATION - Maintain accurate Workload Measurement Statistics using departmental guidelines. Participate in program/department Quality Council meetings and hospital and regional committees. When requested act as a mentor/preceptor in the orientation and training of new staff and learners.
- When requested assist Managers to define clinical direction of program/department. If requested develop activity plan for learners and orientation schedule for new employees. Learners include Dietetic Interns and students in Master's Degree programs.
