Who We Are
Since 1973, The Alex has been a community-based, not-for-profit organization providing integrated primary health care, community support services, and housing-focused programs to individuals and families who experience barriers to accessing traditional health, housing, and social service systems. Services are grounded in principles of health equity, low-barrier access, trauma-informed practice, cultural safety, harm reduction, and person-centered care.
HomeBase is a scattered-site Recovery Oriented Supportive Housing program operating through an Intensive Case Management (ICM) model. Participants live independently in apartments throughout the community and receive individualized, flexible supports focused on housing stability, health coordination, system navigation, income and benefits support, crisis intervention, and community integration.
Reporting to the Program Manager, The Registered Nurse (RN) is a key member of a multidisciplinary Intensive Case Management (ICM) outreach team supporting individuals experiencing complex health, mental health, substance use, and housing instability challenges. Working within a Housing First and Harm Reduction framework, the RN provides nursing assessment, health education, advocacy, care coordination, health system navigation, and clinical consultation. The RN collaborates with case managers, healthcare providers, and community partners to support health stability, housing retention, and community integration. Working to full scope within a trauma-informed, participant-centered, recovery-oriented, and strength-based model, the RN applies independent clinical judgment, assessment, care planning, intervention, consultation, and escalation as required to improve health outcomes and facilitate access to community-based supports.
This is a permanent, full-time position.
What You Will Do
Program Philosophy

Uphold the principles of Housing First, Harm Reduction, Trauma-Informed Care, and participant-centered practice.

Provide culturally responsive services that recognize the impacts of colonization, systemic barriers, and social determinants of health.

Support participant autonomy, dignity, self-determination, and recovery-oriented goals.

Apply relevant knowledge of legislation and community systems, including the Mental Health Act.

Follow organizational policies, procedures, and professional nursing standards.

Case Management and Care Coordination

Work to full scope of RN practice, including independent assessment, clinical decision-making, participant education, care coordination, risk identification, and appropriate escalation within professional, organizational, and legislative standards.

Complete nursing assessments and identify emerging or changing health concerns, including physical health, mental health, substance use, medication-related concerns, functional needs, and risks impacting housing stability.

Support participant navigation and access to primary care, addiction medicine, mental health services, and community resources both within The Alex and in the broader community.

Collaborate with interdisciplinary team members to develop, implement, and regularly update participant-centered service plans, health goals, and safety plans, with a primary focus on supporting health stability and its impact on housing stability and community integration.

Participate in case conferencing, care planning meetings, and interdisciplinary consultations to support coordinated care.

Coordinate care with internal and external providers to ensure continuity of services.

Support transitions between hospital, detox, shelter, treatment, and community settings.

Identify barriers to engagement and service access and work collaboratively to address them.
-
Identify risks to health and housing stability and collaborate with participants and team members to address them.
-
Advocate for participant rights and equitable access to health, housing, income, and social supports.

Maintain strong relationships with community partners to support integrated service delivery.
Administrative Responsibilities
-
Complete documentation, assessments, reporting, and data entry in accordance with organizational, professional, and legislative requirements.

Maintain accurate, timely, and high-quality clinical records and case notes.

Track nursing activities, referrals, and follow-up to ensure continuity of care.

Participate in quality improvement, evaluation activities, and ongoing professional development.
Team Responsibilities

Work collaboratively as an active member of a multidisciplinary outreach team.

Demonstrate professionalism, flexibility, initiative, and sound clinical judgment in both independent and team-based work.

Contribute to a positive, communicative, and collaborative team culture.

Provide consultation and support to colleagues regarding nursing practice and participant care.

What You Bring
-
Graduate of an accredited degree program in nursing.
-
Active practice permit with the College of Registered Nurses of Alberta (CRNA).
-
3-5 years recent experience in a primary care setting.
-
Experience working with individuals experiencing homelessness, poverty, mental health concerns, and substance use disorders.
-
Class 5 driver’s license, personal vehicle for business use, clean 3-year drivers abstract, and $2 million liability coverage are required.
-
Minimum of 21 years of age and three (3) years of driving experience is required to drive an Alex fleet vehicle.

Basic Life Support (BLS) within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire.

A satisfactory vulnerable sector criminal record clearance is required.
-
Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is preferred.

Driving on Behalf of The Alex
This position requires the use of an Alex fleet vehicle. Successful candidates must show proof of a class 5 driver's license and a clean 3-year drivers abstract. Candidates must consent to the possibility of an annual driver's abstract review to meet our insurance provider’s requirements. Driving a fleet vehicle at The Alex requires that staff are at least 21 years of age with a minimum of three (3) years’ driving experience. Mileage will be reimbursed in alignment with the CRA guidelines.
