As a Community Mental Health Nurse, you will lead the delivery of high-quality, client-centered mental health and substance use services, empowering individuals and families, strengthening community wellness, and advancing recovery through compassionate, culturally responsive care.
Key Responsibilities
- Provide comprehensive community mental health nursing services in a primary care setting
- Deliver client-centered care to individuals experiencing mental health and substance use challenges
- Perform case management, including intake, assessment, care planning, and ongoing follow-up
- Administer and monitor medications (including injections) and coordinate lab work
- Support clients in maintaining independence, wellness, and treatment adherence
- Collaborate with physicians, social services, and interdisciplinary teams to deliver integrated care
- Facilitate referrals, system navigation, and access to community and external supports
- Respond to mental health crises and provide timely interventions and counselling
- Deliver outreach services, including home visits and travel within the region
- Provide psychoeducation to clients and families on mental health, substance use, and medications
- Develop and implement individualized care plans using a recovery-oriented approach
- Participate in discharge planning and ensure continuity of care post-hospitalization
- Promote mental health awareness, harm reduction, and suicide prevention initiatives
Key Qualifications:
- A Baccalaureate degree in Nursing from a recognized university with additional training in mental health or psychiatric nursing/Diploma in Psychiatry Nursing
AND
- Three (3) years of experience in Mental Health or Psychiatry
Equivalencies:
- Licensed practical nurse (LPN) / Registered practical Nurse ( RPN)
- With completion of post-basic courses in psychopharmacology, psychosocial rehabilitation, or an LPN Mental Health Nursing certificate program
AND
- Five (5) years of working experience in mental health
