Position Summary

Under the guidance of the Service Coordination Lead, Children and Families, the Children and Family Resilience & Enrichment Care Coordinator plays a key role in supporting families by connecting them to culturally relevant resources, programs, and services. This position focuses on navigation and coordination, ensuring families have access to wraparound supports that promote resilience, wellness, and enrichment. The Care Coordinator works collaboratively with case management teams, health and education departments, and cultural support workers to help families overcome barriers and strengthen their well-being. This role emphasizes relationship-building, advocacy, and cultural safety while supporting families in achieving their goals.
This is a Full-Time Term Contract with an end date of March 31, 2027, with the possibility of extension.
What you'll do
Core Responsibilities
- Maintain confidentiality of all acquired information in accordance with privacy standards.
- Provide navigation and support in a non-judgmental, culturally sensitive, and trauma-informed manner.
- Deliver services promptly, respectfully, and equitably, following Seabird Island Band’s policies and procedures.
- Uphold fairness and unbiased practices in all interactions with families and service providers.
- Remain accessible, accountable, and responsive to the needs of community members and internal teams.
Navigation & Resource Coordination
- Respond to requests from families, community members, and/or case management teams to identify and connect appropriate services.
- Assist families in understanding available resources and pathways to support resilience and enrichment.
- Support families in clarifying goals and accessing culturally relevant programs without assuming direct case management responsibilities.
- Promote coordinated care by liaising with internal departments and external service providers.
- Maintain healthy professional boundaries while fostering empowerment and independence.
Program Awareness & Community Engagement
- Share information about workshops, educational sessions, and community programs that promote wellness and resilience.
- Identify service gaps or emerging needs and communicate findings to the Service Coordination Lead, Children and Families and Health Team for follow-up.
- Attend community events, forums, and service fairs to strengthen relationships and resource knowledge.
Reporting & Administrative Duties
- Maintain accurate records of navigation activities, resource mapping, and service contacts in compliance with confidentiality standards.
- Submit monthly activity reports to the Supervisor by the 5th of each month.
- Participate in scheduled team meetings with Health, Mental Health, and other departments as required.
- Follow all relevant protocols and agreements, including culturally grounded frameworks and child and family protocols.
Other Duties
- Additional responsibilities as needed to support the community and uphold our shared values.
Working / Special Conditions
- Initial and ongoing employment is subject to submission of a clear Criminal Record Review Program to ensure the safety and trust of the communities we serve. This check will be conducted every five years.
- Initial and ongoing commitment to the values and guidelines of Sq'éwqel (Seabird Island Band), including our policies, procedures, standards of practice, and laws, is essential. This commitment supports our collective work, the community, and the responsibilities of this role.
- Must possess a valid class 5 driver's license and have access to safe and reliable transportation, insured for community-based use, to effectively engage and support projects, work, and initiatives
- Flexibility in working hours may be required, including evenings and weekends, to accommodate the needs and schedules of clients and community needs. Employees will be given as much notice as possible to any changes in schedule.
What you bring
Qualifications
- Post-secondary education in Social Work, Human Services, Indigenous Studies, Psychology, or a related field (Diploma or Degree preferred).
- Training in trauma-informed care, cultural safety, and Indigenous worldviews.
- Knowledge of privacy legislation and confidentiality standards.
- Minimum 2–3 years experience in service navigation, case management support, or community-based programs.
- Familiarity with health, education, and social service systems, including referral processes.
- Strong understanding of wraparound service models and integrated care planning.
- Awareness of colonial impacts, intergenerational trauma, and systemic barriers affecting Indigenous families.
- Organizational and coordination skills to manage multiple referrals and service linkages.
- Advocacy skills to ensure culturally safe and timely service delivery.
- Problem-solving and adaptability in dynamic, client-centered environments.
- Documentation and data management skills for accurate record-keeping.
- Relationship-building skills to foster trust with internal teams and external agencies.
- Cultural humility and commitment to decolonized practices.
- Empathy and respect for diverse lived experiences.
- Collaborative mindset for team-based care planning.
- Discretion and integrity in handling sensitive information.
- Flexibility to attend community events, meetings, and training as needed.
Preferred:
- First Nations candidates are strongly encouraged to apply.
- Lived experience as a member of a First Nations community.
- Knowledge and understanding of Seabird Island’s culture, traditions, teachings, community dynamics.
- Ability to respond quickly and professionally to community or colleague needs and inquiries.
- Excellent writing, editing, charting and verbal communication skills.
- Proven experience in implementing and managing family care plans.
- First Aid/CPR certification (asset).
Cultural Competency:
- Deep understanding of Indigenous cultures, traditions, and contemporary issues.
- Understanding of intergenerational trauma and its impacts on Indigenous peoples and communities.
- Experience working within Indigenous communities and applying culturally safe practices.
