We are currently seeking a Client Care Coordinator who is organized, compassionate, experienced, driven, and dedicated to helping seniors and individuals remain safe, supported, and independent in their own homes and locations.

Responsibilities:
- Conduct client assessments and develop individualized care plans
- Coordinate and oversee home care services
- Build strong relationships with clients, families, caregivers, and healthcare professionals
- Complete client visits, follow-ups, and quality assurance checks
- Assist with care transitions and service adjustments
- Maintain accurate documentation and charting
- Support and mentor caregivers with charting and daily duties while promoting a positive team culture
- Help ensure every client receives the compassionate, reliable care they deserve
Qualifications:
- Healthcare, Social Services, Rehabilitation, Nursing, or related education preferred (2 years preferred)
- Equivalent experience in home care, care coordination, case management, healthcare administration, or senior support services will be considered
- Experience working with seniors, dementia, individuals with disabilities, and family caregivers is an asset
- Strong communication, English spoken fluently with no-barriers, organization, and problem-solving skills
- Experience living and/or working in Northern, rural, or remote Canadian communities is strongly preferred
- Comfortable traveling to rural and remote locations, including farming communities, Indigenous communities, and otherwise hard to reach out-of-town areas
- Familiar with the unique challenges of Northern travel, including winter driving, gravel roads, changing road conditions, limited services, and extended travel times (time and travel always compensated for).
- Must be confident working independently and alone or directing caregivers and or a small group of caregivers in remote settings and willing to travel as required to support clients and families
- Experience navigating rural roads and adapting to varying weather and environmental conditions is considered a strong asset. This is the North where it snows 6-8 mo's a year. We need reliable workers and reliable directives of care.
- Valid driver's license, reliable transportation, and satisfactory Criminal Record Check
