We are currently seeking experienced and compassionate Wound Care Nurses to join our growing team and provide high-quality nursing care to clients in their homes and community settings.
Position Summary
The Wound Care Nurse is responsible for conducting wound assessments, implementing evidence-based treatment plans, monitoring healing progress, and providing client and caregiver education. The successful candidate will work independently in the community while collaborating with physicians, nurse practitioners, and other healthcare professionals to optimize patient outcomes.
Key Responsibilities
- Perform comprehensive wound assessments and ongoing reassessments.
- Develop and implement individualized wound care treatment plans.
- Provide wound dressing changes and advanced wound care interventions.
- Assess and monitor pressure injuries, surgical wounds, diabetic ulcers, venous ulcers, arterial ulcers, skin tears, and other complex wounds.
- Monitor wound healing progress and document findings accurately.
- Identify signs of infection or deterioration and communicate concerns promptly.
- Educate clients, caregivers, and families on wound management and prevention strategies.
- Collaborate with physicians, nurse practitioners, therapists, and other healthcare providers.
- Maintain accurate and timely clinical documentation.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
- Minimum 2 years of recent nursing experience.
- Previous wound care experience required.
- Strong clinical assessment and documentation skills.
- Ability to work independently in community settings.
- Excellent communication and interpersonal skills.
- Valid Ontario Driver's Licence and access to a reliable vehicle.
- Current CPR certification.
- Vulnerable Sector Check.
Preferred Qualifications
- Advanced Wound Care Certification or equivalent wound care training.
- Experience in home care, community nursing, long-term care, or clinic-based wound management.
- Experience with negative pressure wound therapy (NPWT), compression therapy, and chronic wound management.
- Knowledge of best practice guidelines for wound prevention and treatment.
