Functions in an advanced clinical practice role within an interdisciplinary team of chronic disease specialists, in the assessment, management and education of patients and families affected by heart disease. Much of the Cardiac Rehab Nurses role it to assess, monitor and guide patient’s treatment plans as indicated in consultation with physicians and as indicated for orders. The clinician also acts as a resource clinician to health professionals in various settings across the health care continuum, to deliver education and facilitate knowledge translation for evidence guide heart disease care. Participates collaboratively as part of the Chronic Disease Management Program across the South Zone Team. As an advocate for primary and secondary prevention activities will work with a variety of AHS programs and community stakeholders in the prevention and screening of vascular risk and cardiac disease.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Minimum of 3-5 years recent cardiac nursing experience preferably with evidence of working with cardiac patients in the education and management of cardiovascular disease and vascular protection required.
Preferred Qualifications:
Baccalaureate degree in nursing. Experience within the area of Chronic Disease Management in the community setting preferred. Connect Care experience.
