JOB SUMMARY
The Pharmacist provides optimal pharmaceutical care and service for all patients within our Long-Term Care and retirement home servicing pharmacy. The Pharmacist ensures the proper functioning of services and activities in the field of pharmacy and must comply with the professional Code of Conduct, the Pharmacy Act, and all other applicable provincial and federal regulations. This position may be remote, onsite, or hybrid.
PATIENT CARE
- Greet patients and residents in a professional and welcoming manner, building trust-based relationships by name where possible
- Make educated, evidence-based decisions during consultations, clearly documenting all decisions and discussions for continuity of care
- Actively seek opportunities to provide expanded pharmaceutical services through POP systems and health programs
- Engage in regular medication reviews (MedsChecks) to build rapport, ensure compliance, and improve resident health outcomes
- Collect and maintain up-to-date patient records including lab results, clinical measures, and intervention notes
- Identify drug therapy-related problems (e.g., inadequate dosage, adverse interactions) and provide appropriate pharmaceutical interventions
- Initiate or adjust prescription-based drug therapy and ensure timely follow-up with patients and care teams
- Collaborate daily with nursing staff, physicians, and other healthcare professionals to ensure continuity of care in LTC and retirement settings
- Coordinate daily with pharmacy team to ensure timely completion of clinical workflow items in Kroll
- Adapt prescriptions to improve patient care and convenience with proper consent and understanding
- Provide effective prevention and management for any medication incident or adverse event
- Continuously seek quality improvements and report ideas to management
- Enforce and comply with the company’s Confidentiality Agreement
QUALIFICATIONS & SKILLS
- Bachelor of Pharmacy (B.Pharm.) or Doctor of Pharmacy (Pharm.D.)
- Valid Ontario practice license in good standing with OCP
- Knowledge of Kroll
- Exceptional patient and customer service skills
- Excellent clinical decision-making skills, including under pressure

- Excellent listening, empathy, and problem-solving abilities
