The Role: Case Manager- Hubs
This role focuses on reducing risk, promoting positive youth development, and helping youth build a sense of identity, belonging, and supportive relationships. The Case Manager works with youth who are at risk of, or currently experiencing, sexual or other exploitation. This is a hub-based role, with a balance of time spent between case management and attending youth programming to connect with youth, foster relationships, and support their engagement in peer-based activities. The Case Manager collaborates with Trellis Hub staff and community partners to identify and engage youth and their natural supports, manage a caseload, and support youth in goal setting and achieving personal development outcomes.
This is a Monday to Friday role and the working hours will be predominantly afternoons and evening work with occasional weekends.
Key Tasks & Responsibilities:
Client Relations

Build safe, trusting, and supportive relationships with youth and their natural supports.

Provide non-judgmental, client-centered, and strength-based guidance.

Facilitate challenging conversations and support youth in setting and achieving goals.

Connect with and engage youth based on program criteria and respond to crises as needed.
Case Management
-
Support youth in life skills, education, employment, safety, and basic needs.

Work with youth and families to manage conflict, negotiate boundaries, and strengthen relationships.

Connect youth to appropriate services, including culturally safe and inclusive supports.

Document goals, progress, and case notes; complete required forms and reports.

Contribute to program planning and deliver psycho-educational content focused on exploitation prevention and diversion.
Youth Programming Support

Attend Hub programming regularly to meet youth and build relationships.

Collaborate with Hub staff to support youth identified as needing additional support.

Support youth in practicing healthy peer relationships and engagement.

Mentor, supervise, and encourage participation, integrating caseload youth into activities.

Knowledge and Experience Expectations

Degree or diploma in Human Services or a related field, and experience working with children, youth, and families. Equivalencies based on a combination of experience, competence and/or personal suitability will be considered.

Minimum of 2 years’ experience working with vulnerable youth.

Able to work flexible hours including regular evenings, occasional weekends and in various environments

Clear Criminal Record Check with vulnerable sector search and Intervention Record Check required.

Valid Alberta driver’s license & access to a vehicle. Ability to work occasional weekend events.

Experience managing a case load of youth at risk of various forms of exploitation an asset
Reporting Relationship
This position reports to a Manager of Community & Wellness.
