Position Summary
We are seeking a highly motivated and experienced Registered Nurse to join the Renal Program as a Renal Modality Education Nurse.
The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards.
The Renal Modality Education Nurse is an integral member of the interprofessional team responsible for supporting patients with advanced CKD to make informed decisions regarding renal replacement therapy options. This role focuses on patient education, care coordination, and facilitation of timely access to dialysis and transplant pathways.
The role of the Renal Modality Education Nurse includes but is not limited to:
- coordinating and delivering comprehensive education to patients and families on renal replacement therapy options, including home dialysis (peritoneal dialysis, home hemodialysis), in-centre hemodialysis, kidney transplantation, and conservative care.
- supporting patients in informed decision-making aligned with their values, preferences, and clinical needs.
- conducting focused nursing assessments to support modality planning and readiness for renal replacement therapy.
- coordinating referrals and diagnostic work-up for transplant assessment and dialysis access planning (e.g., vascular and peritoneal access).
- collaborating with nephrologists and the interprofessional team to develop individualized care and transition plans.
- facilitating timely access to therapies, including home dialysis and transplant pathways.
- acting as a key resource and point of contact for patients, families, and care providers across care settings.
- supporting seamless transitions between MCKC, dialysis programs, and transplant services.
- participating in program initiatives to promote home dialysis uptake, improve transplant access, and enhance patient outcomes.
- contributing to quality improvement initiatives, program evaluation, and data collection as required.
- providing education and mentorship to staff and interprofessional team members related to modality education and CKD management.
Qualifications
- Current registration with the College of Nurses of Ontario required
- Minimum of two (2) years of dialysis experience required
- Demonstrated recent leadership experience in renal or related nursing required , as evidenced through relevant roles, education, and/or professional activities, including but not limited to: Experience in Charge Nurse and/or Team Lead roles
- Completion of additional nephrology or transplant-related education (e.g. courses/certificate, cNeph)
- Active participation in, or leadership of, committees
- Delivery of nephrology or transplant-related educational presentations
- Current BCLS/HCP certification required
- Canadian Nurses Association certification in Nephrology cNeph(C) preferred
- Minimum of one (1) year of recent independent modalities experience preferred
- Minimum of one (1) year of recent transplant experience preferred
- Master’s degree in Nursing, Education, or related field preferred
- Membership with RNAO or another relevant professional association preferred
- Recent experience in patient education, program development, or adult education preferred
- Current CPI and/or GPA training preferred
- Must be able to reliably travel to and attend all Renal Program locations, including but not limited to Multi-Care Kidney Clinic locations, all dialysis sites (including satellite sites), inpatient units, and patient home visits, as required
- Excellent attendance record is required
- A resume outlining experience, further education, and any special courses is required and must accompany the application.
Demonstrated Knowledge, Skills and Abilities
- Advanced knowledge of chronic kidney disease progression and renal replacement therapy options, including home dialysis and transplantation
- Strong patient education and teaching skills,
including the ability to translate complex medical
information into clear, patient-friendly language
- Excellent case management, organization, and
coordination skills with the ability to manage
competing priorities and timelines
- Demonstrated ability to support shared decision-making and patient-centered care planning
- Effective communication and interpersonal skills to support collaboration with patients, families, and
interprofessional team members
- Demonstrated ability to work both independently and as part of an interdisciplinary team
- Strong critical thinking, problem solving, and
decision-making skills
- Experience with quality improvement, data tracking, and program evaluation preferred
- Demonstrated proficiency with computers (e.g., Microsoft Office, electronic medical records, and patient information systems)
- Commitment to ongoing learning, professional development, and continuous quality improvement
