Job Type: Full-time, Schedule: Monday- Friday: 8:30 a.m.- 4:30 p.m.
Salary: $67,384.00 – $79,275.00 per year

Gateway Community Health Centre, located in Tweed Ontario, provides comprehensive primary care, dental services, and allied health support. Our work focuses on health promotion and illness prevention, delivered through a collaborative, inter-professional team.
The Community Outreach- Registered Nurse conducts home visits, identifies socio-economic barriers and facilitates upstream health promotion and prevention services to address health inequities and improve the client’s experience through an integrated system of care and advocates on behalf of the client and/or family for care closer to home. The role provides leadership focused on quality, safety, strengthening access, coordination, and transitions of care. As the primary point of contact for the client and/or family, the Community Outreach- Registered Nurse collaborates with the interdisciplinary care team, conducts home visits, identify patients who can benefit from care in the home, establish plan of care, and identify with the patient and/or family programs and services from within the community and/ or broader sector care organizations to support the client in the home.

Qualifications
· Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse (RN), including liability insurance.
· Nursing experience in community health, primary care, care coordination, outreach, mental health, chronic disease management, rural health, or related community-based settings.
· Demonstrated knowledge of Ontario’s health care system, community support services, and social determinants of health, including barriers related to housing, income, food security, transportation, mental health, addictions, and access to care.
· Experience working collaboratively within interdisciplinary teams and with community and health sector partners to coordinate care and support successful transitions across sectors.
· Strong client advocacy skills with the ability to support culturally safe, trauma-informed, and client- and family-centered care.
· Excellent assessment, critical thinking, problem-solving, and care planning skills, with the ability to work independently and prioritize in a fast-paced environment.
· Experience navigating and connecting clients to community resources, government programs, and allied health and social services.
· Valid Ontario driver’s licence and access to reliable transportation, with the ability to travel throughout the service area.
