Job Summary
We are seeking a compassionate and dedicated Community Nurse to provide high-quality nursing care within the community setting. The ideal candidate will have a strong background in hospital and outpatient care, with experience in acute, hospice, pediatric, and case management nursing. This role involves working closely with patients and their families to promote health, manage chronic conditions, and deliver personalized care in diverse environments. The Community Nurse will play a vital role in improving patient outcomes through comprehensive assessment, education, and support.
Responsibilities
- Conduct home visits and community-based assessments to evaluate patient needs and develop individualized care plans
- Administer medications, treatments, and injections as prescribed by physicians
- Monitor patient health status and document progress accurately in medical records
- Educate patients and families on health management, medication usage, and preventive care strategies
- Coordinate with healthcare teams to ensure seamless delivery of services across different providers
- Provide emotional support and counseling to patients dealing with chronic illnesses or end-of-life care
- Assist with discharge planning and follow-up care for hospital patients transitioning to home or community settings
- Maintain compliance with healthcare regulations, safety protocols, and confidentiality standards
Requirements
- Valid nursing license (RN or RPN) with current registration
- Prior hospital experience preferred; experience in acute care, hospice, pediatrics, or case management is a plus
- Strong knowledge of medical terminology and clinical procedures
- Excellent communication skills for effective patient education and interdisciplinary collaboration
- Ability to work independently within a community environment while managing multiple cases effectively
- Compassionate attitude with a commitment to providing personalized patient-centered care

Pay: $25.00-$58.54 per hour
