About the Role
We are seeking a compassionate and reliable Community Disability Practitioner to join our team at our Hawkmount Group Home in Northwest Calgary.
In this role, you will support Individuals with complex medical, developmental, and behavioural needs, including cerebral palsy, severe developmental delays, dementia, scoliosis, vision loss, and hearing loss. You will assist with personal care, daily living activities, medication administration, community participation, and transportation to appointments and recreational activities while promoting dignity, independence, and quality of life.
What You'll Do
- Provide personal care and hygiene support.
- Administer medications and follow care plans.
- Assist with meal preparation, housekeeping, and daily living activities.
- Support Individuals in community and recreational activities.
- Transport Individuals to appointments and programs.
- Implement behavioural support strategies.
- Maintain accurate documentation and reports.
- Advocate for Individual goals, choices, and well-being.
Schedule
Awake Overnight Shift
- Thursday to Sunday
- 11:00 PM to 7:00 AM
Qualifications
Required:
- Diploma in Human Services, Disability Studies, Social Work, Psychology, Nursing, Education, Criminal Justice, or a related field (equivalencies considered).
- Valid Class 5 Driver's Licence.
- Reliable vehicle with a minimum of $2,000,000 liability insurance.
- Emergency First Aid and CPR Level A.
- Strong computer and administrative skills.
Preferred:
- Experience supporting Individuals with developmental disabilities and complex needs.
- Knowledge of medication administration and behavioural supports.
- Training in NVCI, APRP, Positive Behaviour Supports, or related areas.
- Experience with personal care, lifts/transfers, and G-tube support.
