Your Opportunity:
We are looking for a highly skilled and enthusiastic Registered Social Worker to join our Recovery Alberta - Mental Health and Addiction Intensive Outreach and Diversion Team in Medicine Hat. This position will work with individuals with complex mental health and addiction issues and will be responsible for the delivery of a continuum of services within Intensive Outreach. Working both independently and as a member of a collaborative multidisciplinary team, you will provide evidence informed and best practices in crisis intervention, assessment, treatment planning, consultation, referral, case management and short-term treatment. The Social Worker II works in an integrated and collaborative way to provide treatment for clients with addiction and mental health needs as well as individuals who are on a Community Treatment Order. You have knowledge of the different Mental Health and Addiction programs and services, community resources as well as the Mental Health Act. You can balance numerous demands and conflicting priorities, accurately collect, and document information in a timely manner.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:
Previous experience in a Community Mental Health and Addiction setting performing addiction and mental health assessments and treatment. Proven organizational and case management skills. Proven excellent interpersonal skills and ability work effectively within a multi-disciplinary team. Preferred Qualifications:
Two to three years previous experience working in an Addiction and Mental Health field preferred. Previous experience working in the community in an outreach program preferred. Connect Care experience preferred. NVCI, Suicide Prevention, naloxone administration, First Aid & BLS training preferred.
