This position is part of the Primary Care Alberta (PCA) South Public Health Nursing department and is located at or out of Brooks public health. The Public Health Nurse (PHN) is an integral member of the PCA Public Health (PH) team, delivering integrated, strength-based, family-centered public health services in clinic and outreach settings. This role supports the South PH program and other public health initiatives, providing holistic care to individuals, families, groups, and communities with a focus on diversity and the social determinants of health. Key PHN responsibilities: Effectively utilize the nursing process to provide safe, ethical and patient/family centered care aligned with PCA’s vision and values. Deliver services across the continuum of PH care including: Perinatal, Postpartum Caregiver/newborn and Well Child programs: health assessment, health screening and planning, health promotion and teaching, supportive health counselling and anticipatory guidance, breastfeeding support - through home and/or office visits. Immunization programs for routine, seasonal, newcomer and outbreak and communicable disease response (Well Child, school-aged, adult). Communicable disease control & Tuberculosis programs: case management, support and follow-up. Provide advocacy, referrals, linkage, consultation and evaluation to meet broad health needs. Collaborate with multidisciplinary teams, community agencies, and external health/social service providers to coordinate care and ensure informed referrals. Other nursing duties to support programs and services within South PH as required.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Previous experience required within the last 5 years working in a public health nursing unit performing generalist PHN duties supporting perinatal, postpartum caregiver/newborn, well child programs, immunization programs, and communicable disease control and Tuberculosis programs. Experience working with infants, children, adults, families and communities, diverse cultures, & vulnerable populations. Experience in interdisciplinary teamwork and demonstrated capacity for building relationships with community partners. Knowledge, understanding, and demonstrated use of the social determinants of health & health promotion strategies. Ability to be self-directed and to exercise independent nursing judgment as duties must be completed with a high level of independence but also interdependent within a larger team environment. Effective and efficient time management skills.
Preferred Qualifications:
Baccalaureate degree in nursing preferred; however, skills/experience/knowledge relevant to nursing practice in community sector will be considered. Epidemiology and immunology courses, Breastfeeding and/or Prenatal Education experience or certification, CNA Certification in Community Health Nursing, Indigenous Cultural and Sensitivity training.
