Position Summary
We are seeking a dedicated and compassionate Case Manager, who is a strong advocate for and has connections to local Queer and Trans networks. As our Case Manager for new comers to Calgary that identify as 2SLGBTQ+ , you will play a crucial role in ensuring these individuals receive the necessary services and support throughout their re-settlement journey. Your experience providing support to this client group along with your fluency in Queer & Trans terminology will be essential in your Case Management responsbilitilies.
Key Responsibilities
- Accountable for the full client care cycle, including assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s settlement and human services needed
- Conducts the assessments and needs against a comprehensive social determinant of health and settlement service plan management framework.
- Determines 2SLGBTQ+ clients’ needs and requirements by completing intake interviews; determining need for settlement, housing, educational, medical, psycho-social, mental health and/or other external needs; reviewing external expert evaluations, objectives, and plans.
- ·Developing, monitoring, and evaluating case management service plans and progress;
- Facilitating interdisciplinary approaches; monitoring staff performance to meet service plan objectives.
- Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in results.
- Improves results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
Requirements
- A diploma or degree in the social services, RSW prefered
- Preference will be given to a person with 2SLGBTQ+ lived experience
- Previous experience providing support to Queer/Trans clients
- Proven experience faciliting support groups and or workshops
- Connections to local Queer & Trans networks
- Knowledge and understanding of HIV/AIDS hisotry and current realities
- An understanding and appreciation for the challenges of LGBTQ+ immigration and refugee issues
- Fluency in Queer & Trans Terminology and Cultural References
- Community Involvement or Activism within queer/trans spaces would be an asset
- Comfort with Non-Traditional Family Structures and Chosen Family Support
