We are currently looking for a nurse to join the work with the SSRD (somatic symptom and related disorders) pathway. This role will focus on inpatients in paediatrics and neurology who are at high risk of experiencing extended, complex and repeated hospital stays. In addition, you will work as part of the program that provides support for patients and families navigating a somatization diagnosis and treatment options at SickKids. The successful candidate will promote continuity of care, improving health outcomes, cost effectiveness and positive experiences for patients and families.
Here's What You'll Get To Do
Care Coordination:
- Provide child and family-centered case management and discharge/disposition planning through coordinating internal and external resources for each patient and working with children, their families and the clinical team to determine appropriate goals, follow up and transition options.
- Follow patients meeting criteria across the system, including medicine, critical care and subspecialty inpatient units to enhance continuity within and between clinical teams.
- Promote effective and efficient utilization of clinical resources by collaborating with the clinical team to determine appropriate and timely interventions while reducing duplication.
- Assist in developing, implementing, and revising individual care plans through collaboration with the clinical team.
- Participate in treatment planning by attending rounds and case conferences.
o Ensure continuity of plan of care between inpatient and outpatient teams
Patient Care:
- Establish partnerships with children and families focused on determining their needs and preferred goals, strategies, outcomes, existing strengths and resources available to promote self-management
- Conduct comprehensive nursing assessments of patients meeting established criteria and collaborate with the clinical team in synthesizing assessment findings to refine, develop and evaluate plans of care.
Patient and Family Education:
- Provide internal and external resource education to patients and families.
- Support the development and implementation of individualized education plans for patients and families in collaboration with the clinical team.
- Support and educate patients and families to effectively navigate the healthcare system.
Continuous Improvement/ Operational Excellence:
- Proactively identify ways to improve continuity of care, resource utilization and the patient/family experience at SickKids.
- Lead and participate in process improvement initiatives to further goals of the organization as related to enhancing system efficiency, reducing non-value-added inpatient length of stay and improving patient satisfaction.
Here's What You'll Need
- Minimum 3 years of related clinical experience. Experience with acute, complex, neo neonatal and/or paediatric patients is preferred.
- Registered Nurse in good standing with the College of Nursing of Ontario.
- Experience designing, developing and implementing new initiatives and programs.
- Ability to set priorities and manage conflicting demands in order to actively manage a large patient case load.
- Excellent communication and interpersonal skills.
- Exceptional time management and organizational skills.
- Ability to work independently with minimal supervision and within a team.
- Proven ability to handle multiple competing tasks.
