Interior Health is currently seeking a permanent full time Life Skills Worker to join our Health Outreach team under Population Health located at the Community Health & Services Centre located in Kelowna, B.C.
Who are we looking for?
We are seeking a highly skilled and experienced professional with demonstrated frontline experience supporting high-risk and vulnerable populations in community health, public health, or mental health and substance use settings. The successful candidate will bring a strong background in communicable disease prevention and control, including HIV and blood-borne pathogens, reproductive and sexual health, immunization, harm reduction, sexually transmitted infection (STI) assessment, testing and treatment, wound care, and mental health and substance use support.
This role requires an individual who is confident working with clients experiencing complex medical, mental health, substance use, and social challenges, and who can apply evidence-informed, trauma-informed, culturally safe, and person-centred practices in diverse community environments. Candidates must possess exceptional clinical judgment, critical thinking, and the ability to build therapeutic relationships while maintaining professional boundaries in complex and unpredictable situations. The ideal candidate will have proven experience collaborating with interdisciplinary teams and community partners, navigating sensitive and confidential situations, and connecting clients with appropriate health and social services to improve health outcomes.

What will you work on?
This position supports the Health Outreach Team under Population Health. The Health Outreach Team provides services related to communicable disease prevention and control (including HIV and blood borne pathogens), reproductive/sexual health, immunization, harm reduction, diagnosis and treatment of sexually transmitted infections, wound care and mental health/substance use support for high risk and vulnerable populations.
This position works as a member of an integrated team to provide structured support services to clients. Monitors clients in accordance with established treatment objectives and/or care plans; observes client’s behaviours and symptoms, documents observations in accordance with standard procedures and/or providing verbal reports on changes in client’s appearance, behavior, and/or health. Provides support to clients in the development and maintenance of life skills. Demonstrates and models appropriate behaviours and provides support in problem solving and decision-making. Assists clients with activities of daily living. Participates in activities to support skill development. Liaises, monitors and coordinates services with mental health and substance use staff, medical professionals, and other relevant service providers.
Scheduling Information:
Shift times are from 08:30-16:30 hours. Days of work are Monday to Friday.
