As our Client Care Coordinator, you will play a vital role in ensuring our clients receive exceptional care while supporting our caregiver teams. This position requires someone who is organized, compassionate, detail-oriented, and committed to delivering outstanding customer service.
Responsibilities
- Coordinate and schedule client care services while ensuring continuity of care.
- Act as the primary point of contact for clients, families, and staff.
- Build positive relationships with clients and respond promptly to their needs.
- Collaborate with nurses, personal support workers, and management to ensure high-quality service delivery.
- Maintain accurate client records and documentation.
- Assist with staff scheduling and coverage for client shifts.
- Monitor client and staff concerns and communicate appropriately with the management team.
- Ensure compliance with company policies, healthcare standards, and privacy legislation.
- Support the onboarding of new clients and assist with ongoing care coordination.
- Perform other administrative and operational duties as required.
Qualifications
Required:
- Previous experience working in healthcare.
- Experience in home care, community care, long-term care, or a related healthcare setting.
- Excellent organizational skills with exceptional attention to detail.
- Strong verbal and written communication skills.
- Outstanding customer service and problem-solving abilities.
- Ability to prioritize and multitask in a busy environment.
- Proficient with computers and Microsoft Office applications.
- Reliable, dependable, and able to work independently while contributing to a collaborative team.
Preferred:
- Registered Practical Nurse (RPN), Registered Nurse (RN), or Personal Support Worker (PSW) certification.
- Previous experience in scheduling or client care coordination.
- Bilingual (English and French) is considered a strong asset.
- Knowledge of community healthcare services and home care regulations.
- Knowledge of Alayacare Software program
