Position Summary
Carleton North Medical Clinic is seeking a Primary Care Nurse Practitioner (NP) to join its Family Health Team (FHT). The NP will play a key role in delivering collaborative, team‑based primary care focused on patient attachment, continuity, access, and population health outcomes.
Working within the New Brunswick Family Health Team model, the NP practices to full scope while partnering with physicians, nurses, allied health professionals, and administrative staff to provide integrated, coordinated care for rostered and unattached patients across the lifespan.
Core Responsibilities
Comprehensive Primary Care (FHT Model)
- Provide autonomous, evidence-informed primary care services including assessment, diagnosis, treatment, and follow‑up
- Manage acute illnesses, stable chronic diseases, and complex patients as part of a shared care approach
- Order, interpret, and act on diagnostic investigations
- Prescribe medications and nonpharmacological therapies within authorized scope
- Deliver preventive care, screening, and health promotion
- Perform clinical procedures relevant to primary care practice
- Support patient attachment and access initiatives aligned with FHT goals
Interdisciplinary Team-Based Practice
- Collaborate closely with medical, nursing, allied health and administrative colleagues.
- Participate in team huddles, shared care planning, and case discussions
- Coordinate referrals to specialists, community services, and external providers
- Support transitions of care and continuity across the health system
Population Health & Quality Improvement
- Contribute to panel management and population health initiatives, including chronic disease management and preventive care strategies
- Use data and clinical indicators to support quality improvement and access initiatives
- Participate in Family Health Team planning, evaluation, and innovation activities
- Support clinic initiatives aimed at improving patient outcomes, experience, and system integration
Professional Practice & Leadership
- Maintain accurate, timely, and complete documentation in the electronic medical record (EMR)
- Practice in accordance with Nurses Association of New Brunswick (NANB) standards, policies, and scope of practice
- Serve as a clinical resource, collaborator, and mentor within the Family Health Team
- Engage in continuing professional development and reflective practice
Qualifications & Requirements:
Education & Licensure
- Master’s degree in nursing with Nurse Practitioner designation
- Current registration and licensure in good standing with the Nurses Association of New Brunswick (NANB)
- No restrictions on practice
- Canadian Nurses Protective Society (CNPS) liability insurance
Experience
- Experience in primary care, family practice, or community health preferred
- Experience working in team‑based or collaborative care models is a strong asset
Skills & Competencies
- Strong clinical assessment, diagnostic reasoning, and clinical decision-making abilities
- Excellent collaboration and communication skills
- Demonstrated ability to practice both autonomously and within shared accountability models
- Strong organizational skills and attention to documentation
- Proficiency with electronic medical records
- Commitment to patient‑centred, culturally safe, and trauma‑informed care
