General Accountability:
Reporting directly to the Primary Care Network Manager, the Prince Geroge Primary Care Network (PG PCN) Coordinator is responsible for coordinating the operation of the PG PCN through the organization of meetings and facilitating communication and collaboration between partners. The coordinator will support the PCN Manager and PCN Implementation Lead on projects and initiatives within the PG PCN portfolio.
Key Responsibilities:
· Provides administrative support to the PCN Manager, PCN Implementation Lead, PCN Convenor, PCN Implementation Physician Lead and PCN Steering Committee and other committees as assigned on projects and initiatives.
· Organizes meetings; collaborates with partners on agenda setting, prepares and distributes meeting material, minute taking, and identifies, tracks, and follows up on action items.
· Record management and maintains bring forward system
· Supports member and partner engagement activities.
· Other duties as required.
What You Bring:
- A background in Health Care, Health Information Management, Business Administration, or a related field and 1-2 years of experience or an equivalent combination of education and experience.
- Experience with gathering, basic collation and communicating complex information and data.
- Demonstrated experience in effective communications, partner engagement, change management and action planning.
- Skilled at collaborating diverse information from diverse team members to support the preparation of briefing notes, background information, and agendas, for effective meetings.
- Ability to remain flexible in the midst of evolving priorities yet clearly communicate boundaries.
- A passion for improving community health and well-being.
- An understanding of the BC health system, primary care, and client-centred care is an asset.
- Proficiency in Microsoft 365
Working Conditions:
· Monday – Friday 8:30 AM – 4:30 PM, office-based
· Ability to accommodate occasional prescheduled evening meetings
· Ability to travel to meetings within the community
