Our client is seeking a compassionate, skilled, and community-focused Registered Nurse (RN) or Registered Practical Nurse (RPN) to join their growing healthcare team in the New Liskeard area. This unique role combines both community-based nursing and in-clinic patient care, offering a rewarding opportunity to build meaningful relationships while delivering high-quality healthcare services.
About the Role
As a Community & In-Clinic Nurse, you will provide professional nursing care to patients in both clinic and community settings. You will work collaboratively with healthcare providers, patients, and families to promote wellness, support chronic disease management, and deliver exceptional patient-centered care.
Key Responsibilities
Provide nursing assessments, treatments, and follow-up care in both clinic and community environments
Administer medications, injections, immunizations, and treatments as prescribed
Perform health assessments and monitor patient progress
Educate patients and families regarding health conditions, treatment plans, and preventative care
Maintain accurate and timely clinical documentation
Collaborate with physicians, nurse practitioners, allied health professionals, and community partners
Support chronic disease management and health promotion initiatives
Participate in care planning and coordination for patients with complex healthcare needs
Ensure compliance with professional standards, organizational policies, and regulatory requirements
Qualifications
Current registration in good standing with the College of Nurses of Ontario (CNO)
Registered Nurse (RN) or Registered Practical Nurse (RPN)
Valid Ontario Driver's License and reliable vehicle
Current CPR certification
Strong assessment, clinical, and communication skills
Ability to work independently and as part of a multidisciplinary team
Experience in community nursing, primary care, family health, or clinic settings is considered an asset
Proficiency with electronic medical records (EMR) systems is an asset
