Job Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. Responsibilities
- Review and interpret physician orders to ensure accuracy and appropriateness of medication therapy.
- Dispense medications accurately while providing counseling to patients on proper usage and potential side effects.
- Monitor patient medication therapies for effectiveness, safety, and adherence.
- Collaborate with healthcare providers to develop individualized patient care plans, particularly in pediatrics and acute care settings.
- Conduct medication reconciliation during patient admissions, transfers, and discharges.
- Educate patients and their families about medications, health conditions, and wellness strategies.
- Maintain accurate records of all prescriptions dispensed and ensure compliance with regulatory requirements.
- Participate in quality improvement initiatives aimed at enhancing patient safety and care outcomes.
Skills
- Strong knowledge of community pharmacy operations and patient care practices.
- Proficiency in acute care pharmacy services with an emphasis on pediatrics is highly desirable.
- Excellent communication skills to effectively interact with patients, families, and healthcare team members.
- Ability to understand and utilize medical terminology related to pharmacotherapy.
- Competence in medication administration protocols and guidelines.
- Detail-oriented with strong organizational skills to manage multiple tasks efficiently.
- Commitment to continuous professional development and staying current with advancements in pharmacotherapy.
Requirements
- Part A Part-Time or Full-Time Registered Pharmacist (RPh) license with the Ontario College of Pharmacists
- Opioid Use Disorder Treatment certification
- CCCEP-accredited injection certification
- Standard First Aid & CPR/AED Level C certification to administer injections
