Health Care Homes Ltd is a progressive community service provider in Lethbridge and surrounding area. Health Care Homes Ltd. provides residential support and community inclusion to adults with developmental disabilities.
We are currently seeking a knowledgeable, innovative and creative Program Co-Ordinator to join our team. This position provides an opportunity to work within a small but dynamic leadership team while working closely with staff and families to ensure our residents have an excellent quality of life and maximum community engagement and inclusion.
The Role:
Reporting to the Executive Director, you will play a pivotal role in leading, mentoring and managing staff to provide high quality and meaningful supports. You will coordinate intakes and design and initiate individual support plans and goals. You will coordinate, monitor and review programming. Provide supervision and direction of personnel in consultation with the Executive Director.
Key Responsibilities/Typical Activities:
To coordinate, monitor and review care/outcomes for individuals.
· Developing Individual Support Plans.
· Full supervision of Programs and staff for day-to-day operations of residences.
· Assist with shift scheduling
· To design and/or approve Planned Positive and Planned Restrictive Procedures
· Coordinate monitoring of Individual Support Plan, Timelines and required reviews.
· Coordinate Functional Behavioral Assessments.
· Data collections, program planning, reports
To help initiate and maintain an effective team building process.
· Demonstrate leadership ability to avoid grievances.
· Maintain an effective communication format with all staff, ensuring pertinent information is shared and acknowledged.
· Supervise and direct personnel and complete their annual reviews.
· Interview all potential new staff and assist with the selection.
· Maintain liaison, communication and effective working relationship
with individuals, guardians, staff and available community resources
· Ensure organization and maintenance of personal equipment and materials.
· On-Call for all programs daily and every other weekend.
· Assist Executive Director with preparation for CET Accreditation
Competencies:
· Certificate/ Diploma/ Degree in Human Services/ Disability Support/ Health Care/ with several years’ experience.
· Strong interpersonal, leadership and communication skills
· Robust conflict resolution and mediation skills
· Proficiency in Microsoft Office (excel, Word, PowerPoint etc.)
· Ability to teach and implement on-going staff education
· Strong analytical, strategic thinking, and problem-solving skills

Benefits:
· Monday to Friday 830 am – 430 pm. Occasional evening or weekends may be required
· On- call daily plus every other weekend
