The Nurse Practitioner (NP) focuses on providing primary healthcare for a roster of clients at the Centre. The Nurse Practitioner will function with an integrated team of professionals that offer primary care, including assessment and development of treatment plans in collaboration with the client, caregiver, and referrals as required. Position Summary
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team. The NP will provide primary health care services to individuals and families, with a strong emphasis on disease prevention, health promotion, chronic disease management, patient education, and community development. Specific functional areas of responsibility include (but are not limited to) health assessment, diagnosis, therapeutics (pharmacological, complementary, and counselling interventions), and the development of treatment plans in collaboration with the client, family, and other GRCHC staff as required. The NP will work collaboratively within an interdisciplinary team, ensuring high-quality, patient-centred care that aligns with GRCHC’s mission and values.
Key Responsibilities:
- Conduct comprehensive health assessments, diagnosis, treatment, and management of acute and chronic conditions.
- Develop and implement individualized care plans in collaboration with patients and interdisciplinary team members.
- Prescribe medications and other therapeutic interventions in accordance with scope of practice.
- Provide health education, counselling, and support to patients and families to promote overall well-being.
- Collaborate with physicians, social workers, dietitians, and other health professionals to ensure coordinated care.
- Participate in quality improvement initiatives and health promotion programs within the community.
- Maintain accurate electronic medical records and documentation in accordance with regulatory and organizational guidelines.
Qualifications & Requirements:
- Registered Nurse (RN) with extended class certification (NP) in good standing with the College of Nurses of Ontario.
- Preferred 2 years of experience in primary care, community health, or a related field.
- Strong clinical skills with a focus on patient-centred, evidence-based care.
- Excellent communication, collaboration, and problem-solving abilities.
- Ability to work independently and within an interdisciplinary team environment. Experience working with diverse and vulnerable populations is an asset.
- Proficiency in electronic medical records (EMRs) and documentation.
- Demonstrated experience in Community and/or Primary Care settings.
- Ability to work autonomously as part of a multi-disciplinary team
- Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and harm reduction Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
- Experience working with electronic medical records (EMR)
This ONA position offers a competitive wage range, of $55.31 to $66.67 per hour and benefits (including HOOPP) are offered.
- Employment is conditional upon providing a police vulnerable sector check and having had a full series of COVID-19 vaccinations
- Hours of work– primarily days; to include some evening, weekend work as required.
