Job Summary
We are seeking compassionate and dedicated Community Support Workers, Registered Care Aides, and Human Services Workers to provide personalized assistance to individuals in our staffed home and out in community settings. This role involves supporting individuals with diverse abilities, and those requiring assistance with activities of daily living. Client needs and care are individualized and may range in complexity.
The ideal candidate will have experience in developmental/diverse abilities care, personal care assistance, behavioral de-escalation, community experience, and a proven calm approach to care — with a strong focus on promoting independence, safety, and well-being, and a clear understanding of behaviors and de-escalation processes.
This is a staffed residential home in the community, with care provided on 8-hour shift rotations — days, evenings, and overnights.
Duties & Responsibilities
- Provide direct, personalized support to individuals with diverse abilities in accordance with each client's individual care/support plan
- Assist clients with activities of daily living (ADLs), including personal hygiene, bathing, dressing, toileting, feeding, and mobility support
- Support clients in community settings — appointments, recreational activities, errands, and social/community inclusion outings
- Apply behavioral de-escalation techniques calmly and consistently; recognize early warning signs and respond proactively to prevent escalation
- Promote client independence, choice, and dignity while ensuring physical and emotional safety
- Administer or assist with medications per policy and training (if applicable/certified)
- Complete accurate documentation each shift — progress notes, incident reports, communication logs — per agency and regulatory standards
- Order groceries and plan/prepare meals for client(s), accounting for dietary needs, preferences, and any nutritional guidelines in the care plan
- Share responsibility, alongside other staff, for the continual upkeep and cleanliness of the home — including laundry, dishes, general tidying, and light cleaning — so the home is consistently maintained across all shifts
- Communicate effectively with the internal care team, family members, and other stakeholders regarding client status and changes in condition
- Follow all health and safety protocols, including infection control, safe transfers/lifts, and emergency procedures
- Attend mandatory training, team meetings, and shift handovers as scheduled
- Work assigned 8-hour shifts on a rotating basis across days, evenings, and overnights, including weekends and statutory holidays as required
- Overnight shifts are sleep shifts: staff remain on-site at all times and are provided their own private room for sleeping during the overnight rotation, waking as needed to respond to client needs
Requirements & Qualifications
- Relevant experience providing developmental/diverse abilities support or community living services (e.g., with organizations such as AxisCare, Connect Communities, or similar community-based support providers)
- Experience with personal care assistance and activities of daily living support
- Demonstrated experience with behavioral de-escalation and a calm, patient approach under pressure
- Certification as a Health Care Assistant/Care Aide, Community Support Worker, or Human Services Worker diploma (or equivalent combination of education and experience)
- Current First Aid/CPR certification (or willingness to obtain)
- Non-Violent Crisis Intervention (NVCI) or equivalent de-escalation training considered a strong asset
- Clean Criminal Record Check, including vulnerable sector search
- Valid driver's license and reliable vehicle considered an asset (for grocery runs and community outings)
- Comfortable with basic meal planning/preparation and routine housekeeping tasks
- Ability to work a rotating shift schedule, including days, evenings, and overnight sleep shifts on-site
- Strong communication, documentation, and teamwork skills

Compensation & Schedule
- Wage: $27.25 – $29.00/hour, based on experience and qualifications
- Shift length: 8-hour shifts
- Shift types: Days, Evenings, Overnights (sleep shifts)
