Job Summary:
The Rexdale Community Health Centre (RCHC) is a multi-service centre that provides primary health care, health promotion, and community support services, including settlement and community development activities for families, children, youth, adults, and seniors of diverse backgrounds. RCHC is recruiting for a part-time Nurse Practitioner to support the work of the Interprofessional Primary Care Team.
The Nurse Practitioner will join our dynamic inter-professional team in providing holistic, comprehensive primary health care services to individuals, families, and communities in the North Etobicoke area. A Registered Nurse, Extended Class, in good standing with the Ontario College of Nurses, will use advanced assessment, physical examination, therapeutic and diagnostic skills to treat, counsel or refer clients. Using a collaborative, shared care approach, the Nurse Practitioner will work with other team members in health promotion and disease prevention, which may include individual, group, or community interventions.
The Nurse Practitioner functions as a member of the primary care team in the provision of primary health care services to residents of the community, with a strong emphasis on illness and injury prevention and health promotion. The Nurse Practitioner will work within the interdisciplinary team at the Centre to address individual and community health issues, including providing holistic assessment of and care for patients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation
A unique aspect of this role is the opportunity to provide care through the Centre's "Wheels to Wellness" mobile health program, delivering primary care services directly within the community and meeting people where they are. The Nurse Practitioner will play a vital role in improving access to care by providing outreach services in community settings and supporting individuals who may otherwise experience challenges accessing health services. In addition, the NP will support the Sickle Cell Disease program.
Responsibilities:
· Provide comprehensive, client-centred primary health care to individuals and families at the Community Health Centre and through community outreach initiatives, including the Wheels to Wellness mobile clinic.
· Conduct comprehensive health assessments; diagnose and manage acute and chronic illnesses; develop and implement evidence-informed treatment plans; prescribe medications; and order, perform, and interpret laboratory tests, diagnostic imaging, and other investigations within the Nurse Practitioner scope of practice and College of Nurses of Ontario standards.
· Provide comprehensive primary care services across the lifespan, including health promotion, illness prevention, chronic disease management, prenatal and postnatal care, newborn assessments, breastfeeding support, and episodic care.
· Deliver specialized primary care for individuals living with sickle cell disease, including comprehensive assessment, ongoing monitoring, management of acute and chronic complications, pain management, preventive care, health education, and coordination of services across the continuum of care.
· Support individuals living with sickle cell disease during transitions between hospital and community settings, including post-discharge follow-up, coordination of specialist referrals, and facilitation of continuity of care.
· Collaborate with physicians, specialists, hospitals, Ontario Health Teams, community agencies, and other health care providers to ensure coordinated, integrated, and client-centred care.
· Develop trusting therapeutic relationships with clients and families by providing culturally safe, trauma-informed, anti-racist, and anti-oppressive care that recognizes the impact of systemic barriers and the social determinants of health.
· Provide health education, counselling, and self-management support to clients and families, with an emphasis on chronic disease prevention, healthy lifestyles, medication management, and disease prevention.
· Assess individual, family, and community health needs and participate in the planning, implementation, and evaluation of health promotion, disease prevention, education, counselling, and wellness initiatives.
· Identify opportunities for innovative and accessible models of care that improve health outcomes and reduce barriers to primary health care for underserved populations.
· Participate in community outreach activities, health promotion events, screening initiatives, and mobile clinic services to increase access to primary care for priority populations.
· Collaborate effectively as a member of the interprofessional primary care team, participating in case conferences, care planning, referrals, and shared decision-making to support holistic client care.
· Maintain accurate, timely, and complete clinical documentation in accordance with professional, organizational, and legislative requirements.
· Participate in quality improvement initiatives, program planning, evaluation activities, and the development of clinical policies, protocols, and best practices.
· Contribute to a positive learning environment by providing mentorship, supervision, and clinical education to students and other learners, as appropriate.
· Maintain professional competence through continuing education, evidence-informed practice, peer consultation, and participation in relevant professional development activities.
· Participate in Centre-wide committees, organizational initiatives, and community events in support of the mission and values of the Community Health Centre.
· Perform other duties consistent with the scope and responsibilities of the position.
Qualifications and Assets:
· Bachelor of Science in Nursing (BScN) and current registration as a Registered Nurse in the Extended Class in good standing with the College of Nurses of Ontario (CNO).
· Current professional liability protection as required by the College of Nurses of Ontario.
· Current certification to prescribe controlled substances, or eligibility to obtain certification.
· Membership with the Nurse Practitioners' Association of Ontario (NPAO) is considered an asset.
· A minimum of three (3) years of clinical experience as a Nurse Practitioner in primary care, community health, or a related setting is preferred.
· Experience providing comprehensive primary care across the lifespan, including health promotion, disease prevention, chronic disease management, and episodic care.
· Experience providing care to individuals living with sickle cell disease, hematological conditions, or other complex chronic illnesses is considered a strong asset.
· Demonstrated knowledge of evidence-informed management of sickle cell disease, including chronic disease management, pain management, care coordination, and health promotion, or a willingness to develop specialized expertise in this area.
· Experience working with interdisciplinary teams using a collaborative, shared-care model.
· Demonstrated experience working with underserved, marginalized, and equity-deserving populations, including Black communities, newcomers, individuals experiencing poverty or homelessness, seniors, youth, and people living with complex health and social needs.
· Demonstrated commitment to anti-racist, anti-oppressive, trauma-informed, culturally safe, and client-centred care.
· Strong understanding of the social determinants of health and their impact on health outcomes, particularly for individuals living with sickle cell disease.
· Knowledge of the Community Health Centre Model of Care and a commitment to health equity and community-based primary health care.
· Demonstrated knowledge of health promotion, illness prevention, chronic disease self-management, and community development principles.
· Knowledge of the Personal Health Information Protection Act (PHIPA), informed consent, privacy legislation, and medical records management.
· Experience using electronic medical records (EMRs); familiarity with NOD, PSS, or comparable electronic medical record systems is considered an asset.
· Strong clinical assessment, diagnostic reasoning, and problem-solving skills with the ability to make sound clinical decisions independently.
· Excellent interpersonal, verbal, and written communication skills, with the ability to build collaborative relationships with clients, families, community partners, and members of the interprofessional team.
· Demonstrated organizational skills with the ability to prioritize competing demands, manage multiple responsibilities, and adapt in a dynamic community-based environment.
· Proficiency with Microsoft Office applications, electronic documentation systems, email, and virtual communication platforms.
· Ability and willingness to work in community settings, including outreach and the Wheels to Wellness mobile clinic.
· Knowledge of the Rexdale community and local health and social service resources is considered an asset.
· Fluency in a second language commonly spoken in the community, including French, Spanish, Arabic, Somali, or another language, is considered an asset.
Salary: $59.39 to $71.56 depending on experience.
Hours per week: 28 hours per week
Duration: 3-year contract position with the possibility of extension.
