Overview:
St. Paul Abilities Network is currently hiring a Community Disability Support Worker for a home in St. Paul to support individuals with complex needs. It is a front-line services role that has the responsibility to support and facilitate the development of skills and competencies that will assist persons with disabilities and complex needs to pursue and fulfill their personal goals and objectives.
Key Responsibilities of the position include:
- Awareness of the client’s physical, social and recreational environments.
- Implement person-centred plans designed to support and assist clients with individual outcomes.
- Transportation to and from community appointments. This could include medical, employment and personal care appointments.
- Assisting and supporting clients in the establishment and maintenance of shared and respectful relationships within the community, with a view towards meaningful participation and inclusion. This includes assisting and encouraging clients to participate effectively in employment, volunteer, recreation and other social settings.
- Understanding of community issues and opportunities and assisting clients with general access and participation in the community.
- Assess the physical environment for hazards and other issues.
- Provide daily progress feedback to supervisors on client individual outcomes. Log progress outcomes and document individual achievements.
- Implement individual behavior management plans.
- When required, participating directly in the care and maintenance of the client’s home living environment.
- Assisting individuals with their personal care requirements.
- Maintain open and professional communication between community partners, families/guardians, peers and supervisors.
- Maintaining clear and objective records, preparing reports and other documentation in a professional and timely manner.
Education and Experience:
Preference will be given to those candidates with a degree, diploma or other advanced training in disability studies, human services, addictions, health care, mental health, or equivalent.
Only applicants with a minimum of one (1) year experience supporting individuals with developmental disabilities and complex needs will be considered for this position.
Applicants must hold a valid Class 5 Driver’s License.
