Job Summary: Under the supervision of the Owner, the Licensed or Registered Nurse LPN/RN is responsible for providing skilled nursing care to clients in accordance with the Plan of Care. The Client Care Coordinator interacts with clients and their families in a variety of settings, while maintaining standards of professional nursing.

Qualifications:
- Graduate of an accredited LPN/RN training program.
- Current registration with Provincial College of Nurses.
- A current First Aid and CPR Certificate (Standard First Aid).
- Vulnerable Sector Police Check.
Responsibilities and Duties:
- Assesses the physical, psychosocial and environmental status of assigned clients in a systematic and continuous manner to provide required client services.
- Uses critical thinking and observation techniques to create the Plan of Care
- Performs ongoing assessment of client’s status and response to services to evaluate the Plan of Care and communicate with the caregiver on any necessary modifications. Plan of Care updates at least every 6 months. More regularly if severity dictates.
- Adheres to Comfort Keepers’ policies, procedures and methods to increase the organization’s effectiveness.
- Continuously evaluates the client’s status and works with clients and their family members, as appropriate, in developing ways to meet their needs. Set up regular check-ins with all clients.
- Assist Human Resources with feedback for 3 month and annual reviews for caregivers.
- Do periodic on-going education on relevant topics for caregivers. Coordinate with HR/Admin to set up.
- Provides those services requiring skills using the nursing process. i.e. (first aid).
- Assists with or administers medications according to Comfort Keepers’ Medication Policy.
- Informs clients of their rights and responsibilities and respects these rights and responsibilities.
- Maintain a high regard and value for the caregivers hired, encourage participation of the caregivers in group activities, communicate well with them, through periodic phone conversations, in office visits, monthly newsletter, setting up events periodically to enhance participation and a feeling that they are cared for.
- Adheres to all Comfort Keepers documentation and timeliness requirements.
- Complies with all Comfort Keepers policies and procedures.
- Participates in in-service programs, team conferences, Quality Assessment/Improvement and committees as required.
- Will be responsible for taking On-Call Phone if necessary.
- Assess client and employee safety and adjust care plans and skills to always allow best practices.
- Handle in-coming prospect calls and book in-home assessments.
- Works to promote the Comfort Keepers brand and company name.
- Assists with improving KPIs for the growth of the company.
- Promotes a safe and healthy work environment free of harassment and abuse.
- Is cognizant of the ways the company can grow and expand.
Pay: $30.00-$30.35 per hour
