This is an opportunity for an LPN to work to their full scope as a Chronic Disease Management Nurse, within the Northwest PCN. The successful candidate will be responsible for providing education and panel monitoring of our Chronic Disease patients. You will work closely with the physicians in our clinic. The successful candidate will be eligible for a northern allowance based on hours of work. You may be eligible for a Northern Residents tax exemption. Northern Allowance: In addition to a competitive rate of pay, Primary Care Alberta currently has a Northern Allowance in place to an annual maximum of $6,300. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay. Travel Expense Reimbursement: Permanent and temporary employees working in excess of one (1) year whose work sites are above the 57th parallel are also eligible to receive an annual reimbursement of personal travel expenses to an annual maximum of $1,235. Eligible employees will be reimbursed for the cost of one (1) round trip, per calendar year, to any destination within Alberta. Travel must originate from a community above the 57th parallel to any destination within Alberta and cannot be carried over.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and PCA policies and procedures. Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
You must have at least four years of experience as an LPN. You must be an excellent team player and a great communicator. You must be self-directed and comfortable creating care plans for your patients. You must be comfortable with teaching individual patients and providing education at group events. You may be required to travel to outlying communities, so you must be comfortable driving on rural roads. You must be comfortable providing care to a diverse population consisting of multiple nationalities and indigenous patients. Successful candidates who have no prior experience will be required to complete and pass coursework related to Diabetes education and other chronic diseases.
Preferred Qualifications:
Familiarity with working within a PCN in the role of CDM nurse. Familiarity with Connect Care is advantageous.
