About Us
Health for All Family Health Team is an inter-professional primary care clinic. We provide a wide range of primary care services including acute, chronic and preventative health care. Our physicians and other health professionals have diverse interests such as palliative care, geriatrics, home visits, low risk obstetrics and global health.
A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for our patients.
Health for All is also the site of the Markham Family Medicine Teaching Unit (FMTU). The FMTU is a partnership between Oak Valley Health–Markham Stouffville Hospital and the University of Toronto Department of Family and Community Medicine and Health for All.
Role Overview
As a member of our Family Health Team, the Registered Dietitian is responsible for the planning, implementation and evaluation of a wide range of patient-centered nutrition counselling services including nutrition assessments and a variety of related health promotion and education programs. The Registered Dietitian will work alongside other members of the inter-professional team to provide and support nutrition interventions to support chronic disease and other care management needs.
*We are currently looking for 2 Dieticians- locations: Markham and Uxbridge
Primary Responsibilities
- Work with our interprofessional team to provide specialized nutrition therapy
- Consult with patients to assess food intake, determine nutritional requirements and monitor progress
- Develop nutrition care plans for patients based on their nutritional information and health history
- Consult with patient’s primary care provider to discuss progress and follow-up
- Perform timely charting and relevant documentation
- May involve home visits or offsite visits to community agencies
- Plan, facilitate and evaluate group education efforts and other health promotion programs on various nutrition topics, such as diabetes and weight management
Qualifications & Skills
- Current licence with the College of Dietitians of Ontario in good standing
- Minimum 1 year relevant experience
- Require professional liability insurance
- Certified Diabetes Educator (CDE) designation is an asset
- Group program (e.g. Healthy Eating) facilitation and other relevant program planning experience is an asset
- Ability to work in an inter-professional team environment
- Ability to prioritize, manage time effectively and be flexible in an active work environment
- Effective communication and interpersonal skills
- Experience in both individual and group counselling, including program planning, facilitation and evaluation
- Flexibility to work some evening and weekend shifts
- Excellent oral and written communication skills
Pay: From $74,000.00 per year
