We are currently seeking an experienced and compassionate Nurse Practitioner to join our growing mobile care team. This role is ideal for a clinician who values relationship-based medicine, enjoys working autonomously, and is passionate about delivering high-quality care directly in patients’ homes.
Position Overview
This is a permanent part-time position with the opportunity to expand to full-time as patient demand and services grow.
The Nurse Practitioner will provide comprehensive in-home medical care to patients across Calgary and surrounding communities. This role includes assessment, diagnosis, treatment planning, chronic disease management, and coordination of care for medically complex and homebound patients.
A strong background in palliative and supportive care is essential, as many patients require symptom management, goals-of-care discussions, and compassionate longitudinal support in the home setting.
The successful candidate will be confident working independently, demonstrate strong clinical judgment, and have a compassionate, person-centered approach to care.
Key Responsibilities
- Conduct comprehensive medical assessments and provide in-home patient care
- Diagnose, treat, and manage acute and chronic medical conditions
- Develop individualized treatment plans and coordinate ongoing care
- Provide palliative and supportive care, including symptom management and goals-of-care discussions
- Support patients with complex medical needs, frailty, and mobility limitations
- Collaborate with physicians, allied health professionals, home care teams, and community resources
- Complete timely and accurate medical documentation within the electronic medical record (EMR)
- Provide patient and family education to support health outcomes and care planning
- Participate in virtual care and interdisciplinary case discussions as needed
Qualifications & Skills
- Current registration and licensure as a Nurse Practitioner in Alberta in good standing
- Demonstrated clinical experience in palliative care is an asset
- Strong assessment, diagnostic, and clinical decision-making skills
- Experience managing medically complex, geriatric, or homebound patients is strongly preferred
- Excellent communication and interpersonal skills, with a compassionate and person-centered approach
- Ability to work independently and manage a mobile patient schedule effectively
- Comfortable working in patients’ homes and community settings
- Valid driver’s license and access to a reliable personal vehicle are required
- CPR certification and professional liability coverage as required by regulatory standards
