We are currently seeking a full-time Patient Care Coordinator to join our Enhanced Services team. This position is fully remote.
The Patient Care Coordinator is the first point of contact for patients accessing services through Resolve Sleep Enhanced Services.
In this important front-line role, you'll be the first point of contact for patients—setting the tone for a positive, supportive experience from start to finish. You’ll help keep the clinic running smoothly, support our clinical team, and play a key role in delivering excellent care.
This role is responsible for:

Guiding patients through their care journey, including scheduling appointments with Nurse Practitioners and ensuring all required forms and documentation are completed prior to appointments

Coordinating referrals and working closely with clinics that refer patients for CPAP prescription support, Home Sleep Apnea Testing (HSAT), Oral Appliance Therapy (OAT), supplements, CBT-I, and other Enhanced Services programs

Facilitating home sleep testing (where applicable), including patient communication, education, scheduling, shipping coordination, and follow-up

Supporting web-generated referrals and helping patients access services such as CBT-I, supplements, sleep consultations, and other virtual care programs

Identifying opportunities for patients to benefit from additional sleep health services and programs, and helping connect them with the most appropriate care pathway

Monitoring patient progress through established workflows and ensuring timely follow-up, appointment completion, and communication between patients, clinicians, Nurse Practitioners, and partner clinics

Maintaining accurate documentation and administrative records within company systems

Acting as a role model and ambassador to the company by living our shared Resolve Sleep Health values
The PCC works closely with clinicians, Nurse Practitioners, clinic teams, and external partners to support timely access to sleep healthcare while providing exceptional customer service and administrative support.

Previous experience in healthcare, administration, or customer service

Comfort with digital tools, including attention to detail and accuracy in EMR systems, documentation and virtual care (training provided)

Bilingual French and English preferred.
