What you will do:
- Guide and support therapy teams in adopting integrated care practices, helping shift practice from discipline-specific service delivery toward collaborative, team-based care with shared goals, coordinated interventions, and a strong focus on patient outcomes.
- Lead service delivery excellence by overseeing patient care quality, resolving client concerns, managing risk, and ensuring compliance with organizational, regulatory, and professional standards.
- Carry a clinical caseload
- Coach, mentor, and support clinical staff through onboarding, performance management, training, and ongoing professional development.
- Monitor team performance, engagement, retention, and utilization while ensuring staff are aligned with role expectations and equipped to deliver high-quality care.
- Support clinical operations through caseload management, chart reviews, audits, incident investigations, documentation oversight, and quality assurance activities.
- Provide clinical guidance, problem-solving support, and education to therapy and care teams, promoting best practices and evidence-based interventions.
- Identify opportunities for continuous improvement and drive operational and clinical excellence across service delivery.
- Participate in recruitment, workforce planning, and staffing coordination to ensure appropriate provider coverage and a positive patient experience.
- Represent the organization with external stakeholders and contribute to initiatives that enhance quality, collaboration, and healthcare outcomes.
What you will need to be successful:
- Possess current registration and be in good standing with the applicable professional regulatory body (Registered Dietitian, Physiotherapist, Occupational Therapist, Speech-Language Pathologist, or Social Worker).
- Have 2 years of supervisory experience and/or experience in the home health service delivery
- Be able to establish and build rapport with clients and Care Staff effectively
- Possess a valid Alberta driver's license with access to a reliable vehicle
- Have the ability to provide the best possible solutions to problems that align with policies and uphold best practices
- Be self-motivated with an ability to work both independently and as an effective member of a team
