Position Overview
In keeping with the philosophy, objectives, policies, and procedures of Hawthorne Seniors Care Community, provides professional Nursing care to residents; offers leadership and assistance to other staff members of the Resident Services Program; and participates in and facilitates interdisciplinary, resident-centered care to achieve desired resident quality of life outcomes.
We are looking to fill two (2) casual positions with individuals that can work a mixture of day, evening and night shifts.
Summary of Role Responsibilities
Reporting to the Director of Care, Resident Services, provides direct care and ensures quality care for residents. Coordinates the day-to-day delivery of care in the LTC Program in both the Lodge and after hours, in the Cottages.
Duties and Responsibilities
1. Performs direct nursing care for residents as required.
2. Coordinates and participates in care conferences, medication reviews and family conferences as required. Assists staff with the assessment and documentation of resident interventions. Provides individual instruction and follow-up to nursing staff, residents, and families as necessary.
3. Coordinates day-to-day resident care activities by performing such duties as coordinating work assignments and monitoring for resident care outcomes. Organizes Care staff to ensure safe care and services, supports and educates nursing and care staff as required.
4. Monitors resident acuity to ensure effective, quality care. Working within prescribed staffing guidelines makes adjustments to team assignments as changes in census and/or acuity occur.
5. Participates in and directs care to residents receiving Peritoneal Dialysis. Ensures all required assessments are completed. Liaises with nurses and physicians in acute care dialysis units when required.
6. Participates in creating and maintaining a safe and healthy work environment by understanding and following health and safety rules, regulations, and practices including fire, disaster, and WHMIS and infection control.
7. Stays current and informed about all Emergency Preparedness and Response plans and protocols.
8. Participates as a front-line leader in providing input to Management on clinical issues which impact resident care.
9. Acts as a resource and a mentor for other team members. Particularly for LPNs to ensure their nursing practice reflects advances to the scope of the LPN role.
10. Completes other duties as assigned.
Qualifications
Education, Training and Experience
· Current registration as a registered nurse with the BCCNM.
· Advanced preparation in gerontological nursing and dementia care
· Five years gerontological experience
· First Aid / CPR
· WHMIS and Violence Prevention
· An equivalent combination of education, training, and experience acceptable to the Director or CEO.
