Position Summary
Reporting to the Manager, Diabetes Education Program, the Registered Nurse (RN) is a key member of the interdisciplinary Diabetes Education Program (DEP) team. The RN provides comprehensive diabetes education, assessment, counseling, and self-management support to adults living with Type 2 diabetes, pre-diabetes, and individuals at risk of developing diabetes.
Working within a client-centered, culturally responsive, and evidence-informed framework, the RN supports clients in achieving optimal health outcomes through individual consultations, group education, chronic disease management, health promotion activities, and community outreach initiatives. The RN collaborates closely with physicians, nurse practitioners, registered dietitians, and community partners to deliver high-quality diabetes care and education.
Key Responsibilities
Clinical Care & Diabetes Management
- Conduct comprehensive health and diabetes assessments, including review of medical history, medications, laboratory results, and self-management practices.
- Develop individualized care plans in collaboration with clients and interdisciplinary team members.
- Provide diabetes self-management education, including:
- • Blood glucose monitoring
- Medication management and adherence
- Insulin initiation and titration support
- Lifestyle modification strategies
- Risk reduction and complication prevention
- Foot care and self-care practices
- Monitor client progress and evaluate outcomes through ongoing follow-up and reassessment.
- Maintain accurate, timely, and confidential clinical documentation in accordance with professional and organizational standards.
Education & Health Promotion
- Plan, facilitate, and evaluate group education programs and workshops for clients with diabetes and pre-diabetes.
- Deliver culturally safe and accessible education that addresses the diverse needs of the community.
- Support clients in setting realistic health goals and building self-management skills.
- Participate in community outreach initiatives focused on diabetes prevention and chronic disease management.
Interdisciplinary Collaboration
- Collaborate with physicians, nurse practitioners, registered dietitians, and other health professionals to provide coordinated client care.
- Participate in case conferences, program planning, quality improvement initiatives, and team meetings.
- Develop and maintain effective partnerships with community agencies and healthcare providers.
Quality Improvement & Program Development
- Contribute to program evaluation, quality improvement activities, and evidence-based practice initiatives.
- Support the development and implementation of educational resources and program materials.
- Remain current with emerging diabetes management guidelines, best practices, and clinical standards.
Qualifications
Education & Registration
- Bachelor of Science in Nursing (BScN) preferred.
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current Certified Diabetes Educator (CDE) designation is mandatory.
- Current CPR certification.
Experience
- Minimum three (3) years of recent clinical nursing experience.
- Demonstrated experience in diabetes education, chronic disease management, and client self-management support.
- Experience working within a community health centre, primary care, or outpatient setting is preferred.
- Experience facilitating group education and community outreach programs is considered an asset.
Knowledge, Skills & Abilities
- Comprehensive knowledge of diabetes prevention, management, medications, insulin therapies, and current clinical practice guidelines.
- Strong health teaching, counseling, motivational interviewing, and behavior change facilitation skills.
- Demonstrated commitment to health equity, cultural competence, and anti-oppressive practice.
- Excellent communication, interpersonal, and relationship-building skills.
- Ability to work effectively both independently and as part of an interdisciplinary team.
- Strong organizational, time management, and documentation skills.
- Proficiency with electronic medical records and Microsoft Office applications.
- Ability to speak additional languages reflective of the communities served is considered an asset.
Working Conditions
- Permanent Full-Time position.
- May be required to work occasional evenings and community outreach events.
- Travel between WellFort locations and community sites within Peel Region may be required.
