Overview
We are seeking a highly motivated and experienced individual to join our team as a Care Team Supervisor. As the Care Team Supervisor, you will be responsible for overseeing and managing a team of inter-professional staff and students by directly providing personal care, training others to provide personal care, and supervising the implementation of those providing personal care. This is a leadership role that requires strong organizational skills, excellent communication abilities, and a deep understanding of personal care rooted in the dignity, autonomy and consent for disabled young adults.
Responsibilities
The Care Team Supervisor will:
- Actively use a Social Model and or Disability Justice lens to respond to disability.
- Respond to disability using an intersectional approach to provide a culturally conscious environment for all staff, students and Team Members.
- Maintain Let’s Grow Hubs Standard of Care Policy, Abuse and Harassment Policy, Code of Content, and more.
- Perform direct care duties for individual team members. Including but not limited to: Toileting, feeding, assisting with physical and mobility requirements (including ceiling lifts, hoyer lifts and manual lifts), and the delegation of these tasks to required staff.
- Carry out and promote positive routines/duties/activities in a cooperative, respectful and flexible manner.
- Promote empowerment, dignity and the right of choice for all team members in every aspect of personal care and recreation.
- Partner with Team Members, staff and students to introduce care plans once rapport and mutual trust has been built.
- Maintain a safe and risk free environment by following organizational guidelines, insurance policies, and care plans.
- Keep records regarding personal care and the health of team members organized and confidential. Examples include: MARS, Seizure Records, Medication Administration Waivers, Incident reporting, etc.
- Partake in the administration and delegation of daily medication administration.
- Assist in enthusiastically delivering recreation program activities, meeting the needs of all team members, implementing appropriate and enjoyable activities in a compassionate, respectful and flexible manner.
- Providing coaching, feedback, and delegation in the moment to staff, students, and volunteers to maintain a seamless and welcoming environment for everyone. This includes providing opportunistic feedback to staff when needed.
- Receive coaching, feedback, and delegation with ease and implement in the moment.
- Train staff on current and future policies and model expectations.
- Participate in development and performance plans of staff.
- No behavioral therapy or modification will take place at Let's Grow. Instead, encourage appropriate behavior through kindness to oneself and others, and effectively support the resolution of challenging behavior of staff, students, and team members.
- Work with team members, family members and other stakeholders in a collaborative and supportive manner.
- Collaborate with the Recreation Program Coordinator to create seamless daily personal care schedules that support the implementation of programming activities.
- Participate in the intake, development, training, and grading of placement students attending Let's Grow.
- Partake in work events, including but not limited to staff meetings, family events, parent-staff touchbases, community events, and outings that may run outside of traditional work hours. These events will be communicated in advance and worked into your schedule.
- Maintain attendance and punctuality to ensure staff to Team Member ratios and the successful operations of Let’s Grow.
- Monitor safe working conditions and follow organizations practices in all areas, reporting potential hazards, incidents, and concerns in a professional and timely manner.
- Keep records of PPE, First Aid Kits, incident reports, and other risk management materials.
- Maintain current knowledge and understanding of organizational procedures and guidelines.
- Train and Develop new staff, students, and volunteers on risk management and safety protocols.
- Create personal care schedules for staff.
- Create PPE, and personal care equipment ordering lists.
Education
- Development Services Worker (DSW) Diploma/Degree, Personal Support Worker (PSW) Diploma/Degree, and or a Diploma/Degree in a related field including but not limited to: Disability Studies, Community Service Worker, Social Service Worker, etc
Qualifications & Knowledge
- Current First Aid and CPR Certification
- Cleared Criminal Record Check / Vulnerable Sector Screening in the last year
- Experience doing Manual and Electric lifts
- Experience with J/G Tube Feeding
- Experience with Medical Record Keeping is an asset
- Experience with Medication Administration is an asset
- Knowledge/experience with Augmentative Communication Devices and assistive tech an asset
- Minimum 1+ years experience working with adults with physical, intellectual and developmental disabilities (Lived experience will be taken into consideration for this role).
- The ideal candidate has 1+ years of experience managing, supervising, and or providing leadership. This experience does not need to be in the helping field as you will be given training regardless.
- The Care Team Supervisor MUST be qualified and comfortable in providing first aid and CPR in the event of an emergency. Anyone with lived experience in this area will be prioritized.
