The Sioux Lookout Area Primary Care Team (SLAPCT) provides primary health care, including chronic disease management, health promotion and disease prevention, to patients in Sioux Lookout Region including the 28 communities in the Sioux Lookout First Nations Health Authority catchment.
Under the Sioux Lookout Area Primary Care Team (SLAPCT) this position will be responsible for the coordination of administrative duties and community engagement for Allied Health Professionals of their Pod. They will work with clients, caregivers, community resources and the Primary Care Team to assist with the provision of assessments and ensure seamless health care system navigation. They will contribute to a culturally sensitive perspective to health care planning. They will build client and community relationships within the Sioux Lookout catchment area, including the 33 First Nations communities.

Salary Range: $62,867 - $84,442

Qualifications:
- Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment).
- Minimum Grade 12 and/or post-secondary education in a health services field or equivalent working experience
- Experience in the use of email, internet and Microsoft Word, PowerPoint and Excel
- Ability to work in a collaborative team approach, including health care providers, clients and family supports.
- Ability to set priorities and meet tight deadlines
- Ability to communicate effectively orally and in writing
- Excellent organizational and time management skills
- Knowledge of the remote/isolated communities and the health priorities of the First Nations communities in the Sioux Lookout Health Authority catchment area.
- The ability to function effectively during periods of rapid change
- Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an
- Must be willing to travel by various modes
- Must have a valid driver’s license
- Must be willing to relocate and live in Sioux Lookout (or within a daily commuting distance)
- Ability to perform the requirements of the role on a regular basis
Roles and Responsibilities:
- Communicate and act as a liaison between the Primary Care Team and the community health team.
- Establish and build relationships with community stakeholders, action community concerns and requests appropriately.
- Assist with the planning and coordinating of the travel and activities of Primary Care Team Allied Health.
- Provide linkage within the local health areas and the communities within the Sioux Lookout region.
- Assist in navigation of barriers that are identified in delivering client care in communities.
- Provide outreach and promotion, conduct presentations to various stakeholders at conferences, fairs, etc.
- Entering of data/visit information into the Electronic Medical Record and Schedule.
- Maintain privacy and confidentiality in the handling of sensitive/confidential material strictly in confidence in accordance with SLFNHA privacy policy guidelines.
- Participate as part of the multi-disciplinary team on clinic days.
- Work closely with clients and communities to ensure clients are seen by the Primary Care Team in a way that is efficient and practical for the clients.
