QUALIFICATIONS:
Successful completion of an approved RPN or RN program
Current registration with the College of Nurses of Ontario
Evidence of strong clinical skill sets
Previous experience with electronic medical records (EMR) an asset
Demonstrated ability to work both independently and effectively in a multi-disciplinary team environment
Excellent communication, organizational, multi-tasking and problem-solving skills.
DUTIES
Assessment of patient health care needs and treatment outcomes. Recording of vitals, patient history, biometric measurement, etc.
Coordination of diagnostic, therapeutic and management plans with the patient and family doctors
Procedures including immunizations, injections, immunotherapy, venipuncture, ear syringing, dressings, assisting in minor surgery, urinalysis, pregnancy testing
Participate in the development and implementation of all of the PSFHT's general and targeted clinical programs, as required.
Help patients to identify and use health resources, involve patients in decisions about their own health, and encourage patients to take action for their own health
Initiate and support health education activities
Liaison with community agencies, hospitals, diagnostic and therapeutic services, pharmacies
Collaboration with other team members to optimize patient health
Provide communication to hospital and community staff and other interdisciplinary team members to ensure smooth transition for care
Referrals to community services, as appropriate
Other duties as assigned.
